Objective: To observe the effects of ovulation induction in stimulation cycles of PCOS by sequential therapy of nourishing-yin and supplementing-yang combined with letrozole (LE) and human menopausal gonadotropin (HMG) and investigate the mechanism to provide thinking and method for the treatment of PCOS with combination of Traditional Chinese Medicine and Western Medicine.Methods:The forty-seven PCOS patients were randomly divided into two groups, the treatment group,26 patients, by sequential therapy of tonifying-yin and supplementing-yang combined with LE and HMG for 34 cycles, and control group,21 patients, by LE and HMG for 30 cycles. To observe the date of HCG-injection, the thickness of endometrium on the day of injection of HCG and the number of the good follicles,the dosage of HMG. And compare the rate of ovulation, pregnancy, OHSS and LUFS between the two groups.Results:(1) The cure rate of treatment and control groups are 35.3% and 13.3% respectively, with effective rate of 82.4% and 60.0%(P<0.05). (2) There were no significant differences on the day of injection of HCG. between treatment and control groups(P>0.05). There were significant differences in the thickness of endometrium on the day of injection of HCG between the two groups(P<0.05). The rate of uni-follicle development cycles of treatment group is 70.6% compared with 23.3% of control group (P<0.05). The amount of follicles with diameter≥14 mm and diameter≥18mm on the day of HCG-injection of treatment groups is significantly less than control group (P<0.05). The dosage of HMG of treatment group is significantly less than control group. The difference is significant (P<0.05).(3)The ovulation rate of treatment and control groups are 82.4%(28/34) and 60.0%(18/30) respectively, with pregnancy rate of 35.3%(12/34) and 13.3%(4/30). There were significant differences between the two group(P<0.05). The rate of OHSS between treatment and control group are 5.9% and 30% respectively. There were significant differences between the two group(P<0.05).Conclusion:Sequential therapy of nourishing-yin and supplementing-yang combined with LE and HMG would increase the cycles of uni-follicle development, thicken the endometrium, raise the rate of ovulation and pregnancy with the low incidence of side-effects (thin endometrium and OHSS). Combination of Traditional Chinese Medicine and Western Medicine would decrease dosage of exogenous gonadotropin. Sequential therapy of nourishing-yin and supplementing-yang combined with LE and HMG is effective to the ovulation induction in stimulation cycles of PCOS.
