Construction Of Early-warning System Of Medical Quality Risk

The competition in medical market is much stronger after the entry into WTO of china. So the development of hospital is facing great challenge. To be the winner of the competition and obtain sustainable development, the hospital should improve the medical quality and management quality to enhance the power in the industry. Quality management is the focus of hospital management, while the medical quality management is an important issue of quality management. To improve the quality management, the hospitals have to exert absolute control and normalization of medical quality. Meanwhile they should establish valuable,available and effective control system of quality management, regulate the quality management process and improve hospital quality to satisfy patients and create greater social profit and economic profit. This is not only market demand, but also the need of development and reforming of medical industry. It has great significant on the construction of harmony society and guarantee the basic health right of citizens.At present, the approach of risk management has been broadly applied in the enterprises and business management activities. It aims to realization the corporation value and maximization the business profit through the preventing,control,avoidance and reduce of risk costs. The medical industry has the nature of social welfare, and its basic purpose is to protect peoples'health and guarantee the social profit. Hence, the main purpose of hospital medical risk management is to reduce various risk factors during the process of medical service and ensure the safety and effectiveness of medical service. Furthermore, for the survival and development of hospital, risk management should try their best to reduce economic lost caused by risk accidents and reduce the risk cost of hospital operation and management. Meanwhile, medical risk management should take account of the indirect costs caused by unnecessary medical accident. To decrease the numbers of medical accidents, hospitals should focus on the active improvement of service attitude and technical quality. In the abroad, parts of the hospitals who use risk management mode always use the medical quality guarantee plan as well. The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) has established standards of hospital evaluation in 1995, which emphasis on that risk management is an important part of medical quality improvement.By reviewing of the current international common used medical quality indexes, as JCIA,IQIP,ISO9000,literatures have been clarified and theory has been discussed. Up to present, the research of the early-warning system for the risk of the medical quality has been carried out both at home and abroad. The majority of these researches aimed at providing theoretical guidance for the scientific decision-making of local health departments. However, the hospital-related research is very limited. Therefore, hospitals are selected as objects in this study. 33 experts from 6 three"A"hospital are selected. And by using a series of internationally recognized quality health care system as a source of reference and focusing on process control and standardizing of management, the early-warning system for the risk of the medical quality has been constructed based on the combination of theory and practice. At the same time, Delphi method has been used to verify the results repeatedly to form a set of early-warning system for the risk of the medical quality which is objective, scientific and approved by experts. To test the result of this study, a hospital was selected to be evaluated of the risk early-warning system. And by the level of evaluated risk, the reason leading to the medical quality risk has been clarified and the improvement approach has been presented.Of course, there are various factors related to the medical quality risk, the most important indicators have been figured out by the survey to construct the early-warning system. The final results aim to provide the possibility of emergence of medical quality risk to administrators and supervisory authorities of medical quality. It can not reflect the timing and degree of medical quality risks also can not be the instrument to comprehensively evaluate the level of medical quality. So there is limitation of the selection of early-warning indicators. As a result, the administrators and supervisory authorities of medical quality should focus on the standardized management of medical quality, investigation of potential risk, preventing illegal operations, improvement of medical quality supervision mechanism, enhancing staff awareness of risk, and effectively improve the quality of care to protect patient rights and interests. Full-text structure is divided into six chapters:The first part is introduction, focusing on the research background, research purposes, research content and research methods. In order to better expose the significance of this study, the focus and innovation of the study are highlighted. So the value of medical quality risk early-warning system in the quality management process can be pointed outThe second part is a theoretical overview of the medical risk early warning research. By defining of the content of medical risk and related concepts, a brief introduction of the study of the evolution of early warning systems has been presented.The third part is the status analysis of medical risk early- warning system in China. Firstly the study introduces the overall situation of risk management of medical quality. And then based on the theoretical study the feasibility and application prospects of medical risk early-warning system has been proposed.The fourth part is the establishment of medical quality risk early-warning system for hospital, including the risk early-warning indicators of medical quality choices, and the determination of weights of risk early-warning indicators.The fifth part is the application of medical quality risk early-warning system, including the division level of risk, and discussion of the choice of response options.The sixth part is inspiration and recommendations. Some feasible strategies and policy recommendations ha been stated to the establishment of hospital medical quality risk-early warning system to put forward.The final part is the conclusions and prospects, based on the summarizing of main research work and conclusions the limitation has been pointed out.
