Objective : Through clinical research to observe the clinic effect of Tangzhiping which can tonify Qi and spleen, nourish kidney and activate blood eliminate stasis and smooth collaterals on early diabetic nephropathy which is the micro-vascular complications of diabetes mellitus. Discuss the mechanism of Tangzhiping on relieving micro-vascular chronic inflammatory reaction and improving vascular endothelial cell function. Provide evidence for traditional Chinese medicine curing early diabetic nephropathy and lay the foundation of TCM treating micro-vascular complications.Methods:Forty patients with phaseⅢDN were randomly divided into two groups: treatment group and control group, and each group has twenty cases .While giving conventional therapy ,the treatment group were given Tangzhiping, and the control group were given Lotensin respectively for 1 month. Observe the clinical symptoms and signs, and measure fasting blood-glucose (FBG), blood-glucose 2 hours after meal (2hBG), glycolated hemoglobin (HbA1c), Urinary Albumin Excretion Rate (UAER),β2-microglobulin(β2-MG), high-sensitivity C-reactice protein (hs-CRP), Nitric Oxide (NO) and Endothelin-1 (ET-1).Result:Compared effect of the syndrome of traditional Chinese medicine, the effective rate of treatment group is 85.00%,the control group is 55.00%,there were statistics differences between the two groups(P<0.05). Compared the two groups before and after treatment, the NO level obviously increased, FBG, 2hBG, HbA1c, UAER,β2-MG, hs-CRP and ET-1 obviously decreased (P<0.05,P<0.01). There were statistics differences between the two groups after treatment on FBG, 2hBG, HbA1c, hs-CRP and ET-1 (P<0.05), and no statistics differences on NO level, UAER, andβ2-MG(P>0.05).Conclusions:1.Tangzhiping can decrease hs-CRP to relieve micro-vascular chronic enflammatory reaction.2.Tangzhiping can improve vascular endothelial cell function.3.Tangzhiping can decrease blood glucose, and reduce excretion of urine micro-albumin.4.Tangzhiping can improve the clinical symptoms and signs on early diabetic nephropathy patients.5.Tangzhiping can decrease UAER andβ2-MG to protect the renal function.
