Objective To investigate the association between the serum brain natriuretic peptide (BNP) and early-stage diabetic nephropathy,and analyse to its clinical significance.Methods This study comprised 64 patients with nonobese type 2 diabetes mellitus and 30 normal control .Acording to urinary albumin excretion rate(UAER), 64 cases were divided into two groups,one with microalbuminuria (DN+)and the other with normoalbuminuria(DN-). The serum levels of BNP and vascular endothelial growth factor (VEGF) were measured using competitive Enzyme Immunoassay kit. Data are expressed as the mean EM. Statistical analyses were performed using analysis of variance and coefficient of correlation (r),and multiple gradual regression.Results The serum levels of BNP were significantly higher(p<0.001) in all diabetic patients than those in normal subjects . The blood concentrations of BNP were significantly increased in diabetic patients with microalbuminuria, compared with in those with normoalbuminuria (p<0.001) . The serum levels of BNP were positively and significantly correlated with UAER(r=0.85, p<0.01)in diabetic nephropathy. The serum levels of VEGF were significantly higher in all diabetic patients and those with microalbuminuria(p<0.001). The serum levels of BNP were also positively correlated with the serum of VEGF in diabetic patients with microalbuminuria(r=0.67, p<0.01). The serum levels of BNP and VEGF were positively and significantly correlated with UAER in diabetic patients with microalbuminuria(' respectively=0.603,0.367, p respectively <0.001, 0.01).Conclusion The serum levels of both BNP and VEGF were significantly increased in patients with early-stage diabetic nephropathy and they were positively correlated. The serum levels of both BNP and VEGF were positively and significantly correlated with UAER in early-stage diabetic nephropathy subject.BNP and VEGF may play an important role in the development of diabetic nephropathy.
