Objectives:To study the efficacy and clinical value of pulmonary surfactant ( curosurf ) replacement therapy in newborn with respiratory distress syndrome (NRDS). Subjects and methods:Sixty neonates with NRDS were divided into two groups, Thirty received curosurf and thirty didn't receive curosurf. The changes of C,OI,RI and (A-a)DO2 were calculated during mechanical ventilation. The mortality and the manifestation on chest X-ray were compared between two groups.Results:1.At 6h, 24h, 48h after receiving curosurf, there were significant differences in C (t= 5.9, 5.3, 4.0),OI(t= 4.34, 5.78, 5.87),RI(t= 3.27, 7.0, 8.4),and (A-a)DO2(t= 3.79, 6.38, 5.38)between the therapeutic group and the control group(p<0.05).2.At 6h, 24h, 48h after receiving curosurf, the severity of NRDS on chest X-ray in therapeutic group was milder than that in control group.There were significant differences between the two groups(t=2.5,2.8,2.1,p均<0.05).3.After receiving curosurf, the mortality of therapeutic group was lower significantly than that in the control group.There were significant differences between the two groups(χ2=5.45, p<0.05).Conclusion:Pulmonary surfactant plays an important role in improving oxygen exchange and pathological changes in the lungs. Pulmonary surfactant can reduce the mortality significantly and has definite effect on neonatal respiratory distress syndrome .
