Study On Pattern Of TCM Identification Of Rheumatoid Arthritis Treated With DMARDS

Objective:To explore the Chinese medical pattern characteristic of rheumatoid arthritis (RA) patients between and after treatment by DMARDS, we hope that we can discover some rules. Methods:According to clinical test method was used to observe 120 in-patients of RA. Collected symptoms and classified syndromes by the questionnaire of prospective study on syndrome distribution of RA patients. By the research on the TCM syndrome and tracing its change among before and after 3 month treatment, we try to find some rules. Results:1. The main TCM syndromes of advanced RA include Bi syndrome cause by hot and dampness blocking, Bi syndrome cause by cold and dampness blocking, their proportion are separately: 28.33 percent, 28.33 percent; After treatment the main syndromes are the deficiency of liver and kidney, Phlegm-blood stasis obstructing channel,the deficiency of qi-blood, Their proportion are separately: 41.67 percent, 38.33 percent, 22.50 percent. 2. There are significant differences with regard to the main symptoms between before and after teatment(P﹤0.05). The main symptoms were wandering pain of joints with localized swelling hand localized cold pain which is aggravated by cold, unimpatient, spontaneous sweating before treatment. There were aching in the loins and knees, pale and sallow complexion fatigue, dizziness, losing hair,anorexia, dry mouth and nose. 3. There are significant differences with regard to the tongue proper and tongue furs between before and after treatment(P﹤0.05). It is reddish tongue and thin and yellow fur or yellow and grease fur or scanty fur before treatment, and pale and cyanotic tongue with white coating after treatment. 4. There are significant differences with regard to the pulse manifestations between before and after treatment(P﹤0.05). The pulses are as well as taut and rapid pulse, as well as splippery and rapid pulse before treatment, and as well as taut or thready and unsmoothing pulse after treatment. 5. There are significant differences with regard to effective syndromes, their proportion are separately: Bi syndrome cause by hot and dampness blocking(90.91%), intermingled heat and cold(75.00%), Bi syndrome cause by cold and dampness blocking(64.71%), the deficiency qi-blood(42.86%), the Phlegm-blood stasis obstructing channel(29.17%), and the deficiency of liver and kidney (29.17%). Conclusion:1. The main TCM syndromes of advanced RA include Bi syndrome cause by hot and dampness blocking, Bi syndrome cause by cold and dampness blocking, After treatment the main TCM syndromes are the deficiency of liver and kidney, the Phlegm-blood stasis obstructing channel, and the deficiency of qi-blood. 2. The DMARDS perform better in these syndromes: Bi syndrome cause by hot and dampness blocking, Bi syndrome cause by cold and dampness blocking, than the deficiency qi-blood, the Phlegm-blood stasis obstructing channel, and the deficiency of liver and kidney.
