Objective:To comparison the efficacy between tiotropium inhalation capsules and ipratropim metered dose inhaler in a 12 week treatment in patients with chronic obstructi ve pulmonary disease.Methods:A randomized clinical trial was conducted in 34 stable to severe patients with COPD.After two week of washout period,34 patients are randomly divided into two groups,and treated by tiotropium(18ug once per day)and ipralropi um(80ug tid)respectively for 12 weeks.and pulmonary function,MMRCand distance between rows for 6min are also observed.Dring the trial, salbutamol which could be used as the medicine to alleviat symptoms,could use antibiotics or temporarily add gluco corticoids or theophylline within 4 days.Results:after 12 weeks treatment,the fluctu ation of FEV1 between Tiotropium Bromide group and ipratropium bromide group during the treatment are:Tiotropium Bromide group is higher than tipratropium bromide group, t=2.405,P<0.05,The difference of FEV1/FVC are:Tiotropium Bromide group is higher than ipratropium bromide group,t=2.666,P<0.05.The difference of MMRC are:Tiotro pium Bromide group is lower than ipratropium bromide group,P<0.05.The differ ence of distance between rows for 6min are:Bromide group is higher than tipratro pium bromide group,t=2.189,P<0.05.After thirty patients using this medicine in the research,only four patients have the slightly xerostomia,respectively two in tiotropium bromide group and ipratropium bromide group.Conclusions:Using Tiotropium Bromide one time per day can fuction more dramastically than using ipratropium bromide three times per day, the former are not only able to improve patient with COPD respiratory function,but also improve the quality of life,which provides a new meath to cure patients.
