Objective:To study on the expression and significance of medullary system antigens (CD13,CD33和CD117) in multiple myeloma(MM).Methods:Research 22 cases of MM which have been final diagnosed by bone marrow celluar morphology, clinical situation, imageology, and serology standards. Extract peripheral blood and bone marrow of the patients, determine medullary system antigens (CD13,CD33和CD117) of mull iple myeloma cells with CD38/SSC gating method and CD45/SSC gating method of flow cytometry(FCM), and do related detections, like bone marrow celluar morphology, DNA content, P170, blood common practice, renal function, lactate dehydrogenase levels,β2-microglobulin, X-(?)y of bond, et al.Results:1. The positive expression and expression of CD13 in preliminary diagnosis, non-remission, progression of disease, relatpse group were obviously higher than those in remission group. The expression of CD13 in IgA group was obviously higher than that in IgG group. The expression of CD13 was significantly and positively related to lactate dehydrogenase levels,β2-microglobulin, the expression of CD33 and CD117.2. The positive expression of CD13 in MM is 45.5%. The expression of CD33 in IgA group was obviously higher than that in IgG group, and it was significantly and positively related to CD13 and CD117, but it was not related to prognosis indexes of MM.3. The expression of CD117 in preliminary diagnosis, non-remission, progression of disease, relatpse group was obviously higher than that in remission group. It showed no significant difference in different immunoglobulin typing and staging, and it was not related to prognosis indexes of MM.4. Most antigens on CMC (circulating myeloma cells) of MM were significantly and positively related to those on MMC (marrow myeloma cells). CMC was significantly and positively toβ2-microglobulin.5. When myeloma cells were very few(0.5%),CD38/SSC gating method can still distinguish from the cells. Conclusion:1.CD13 may can prognosticate progression of MM.2. CD33 may be a new target in the immunotherapy of MM.3. CD117 may can prognosticate progression of MM, and it is significantly and positively related to CD13 and CD33.4. CMC is significantly and positively related to that MMC, and it can directly effect progression of disease, may prognosticate prognosis of MM.5. CD38/SSC gating method has a higher sensitivity and specificity than CD45/SSC gating method. It can improve accuracy of diagnosis in MM, and it may be an ideal detecting method in detecting minimal residual disease of MM.
