Objective: 1.To assess the diagnostic and prognostic value of morphological in multiplemyeloma(MM).2.Based on multiple myeloma patients were detected by flow cytometry andmorphological comparison of bone marrow smears, discussed flow detectiontechnology in MM diagnostic and prognostic significanceMethods: 1.Use of Wright-Giemsa ’ s mixed staining technique, 55 patients with multiplemyeloma were classified as mature, intermediate, immature, or plasmablasticsubtype according to Greipp’s criteria. To analyse the clinical manifestation andprognosis.2. Twenty-eight confirmed MM patients were performed multiple myeloma cellssurface antigens detection using FCM. The relationship between surface antigensof expressions and cell morphological characteristics and prognosis laboratoryindices were analyzed.Results: 1.Following the criteria of Greipp et al ,we identified the frequency of myelomatype as:plasmablastic,4cases ;immature,11cases ;intermediate,17case; andmature, 23 cases. The survival time of plasmablastic plus immature myelomawas significantly shorter than that of mature plus intermediate t myeloma .2. Four percent to seventy-five point four percent of positive cells were measuredfrom all patients using CD45/SSC and CD38+/CD138+ gating method, whileeight percent to eighty-three point five percent of bone marrow multiplemyeloma cells were detected using morphological examination.The percentagesof CD138, CD38, CD56, CD19 antigen positive expression rates were 95.5﹪,89.3﹪, 53.6﹪, 7﹪respectively.Conclusion: 1.The morphological in multiple myeloma is one of the factors affecting theprognosis, but the factors affected the prognosis complications, not just based onone-sided assessment of prognostic factors, but should be a comprehensiveanalysis of patients with morphology, immunology and genetics comprehensiveassessment of prognosis.2. The use of CD45/SSC with CD38+/CD138+gating technique can identify multiplemyeloma cells ,Immature myeloma cells more easily see the positive expressionof CD56;Flow cytometry combined with morphology ,To be helpful to thediagnosis and differentiated diagnosis of multiple myeloma.
