[objective] Under the background of the natural history of chronic HBV infection, we probe the relationship among HBV DNA,HBV M quantitation and hepatic function in patients between 20~40 years old with chronic HBV infection in order to discover the internal correlation.It is known to all,we treat the patients of CHB based on the realization of the natural history of chronic HBV infection.Accordingly,comprehending roundly the natural history of chronic hepatitis B infection can help improve prognosis,implement reason therapy and provide evidence for economic hygienics'evaluation.[materials and methods]1.1 Clinical history source:Collect infectious division's clinic service of yan'an university affiliated hospital in 2008 July-2009 November.There are about 1000 cases in all between 20~40 years old those who have not systematically used antivirus drugs and combined other hepatitis virus infection.All these cases's diagnosis consistent in 2005 December's"prevention and cure guide book for chronic hepatitis B"standardization.1.2 Apparatus:American PE company's Gene AmP5700 quantitation gene check-out system and 1235 Auto DELFIA;Hitachi 7600 wholly automatic biochemistry analyser 7600;Japan Toshiba multicolor B-mode ultrasonic diagnostic equipment.1.3 Cardinal reagent:hepatitis B viral nucleic acid fluorescence detection diagnostic kit provided by Zhong'shan university's da'an gene diagnostic center;TRFIA quantitation check out kit provided by suzhou xinbo biotechnology limited company;ALT relevant reagents provided by,Beijing jiuqiang biotechnology limited company.1.4 Methods:①Take three vessels patients'fasting blood to dectect HBV DNA quantitation,HBV M quantitation,ALT,at the same time carry out B-mode ultrasonic diagnostic.②Statistical treatment:Use SPSS17.0 statitical software to carry out straight line correlation analysis;paired-sample means comparison use t-test,multiple sample means comparison use ANOV,skewed distribution use Wilcoxon rank test;rate comparison useχ2 test.It expresses statistic difference when P<0.05.[result]1 Basal data of patients between 20~40 years old with chronic hepatitis B virus infection1.1 Distribution of patients with chronic hepatitis B virus infection:after statistical analyzing, the patients between 20~40 years old with chronic hepatitis B virus infection in immune tolerance,immune clearance,inactive HBV carrier state,HBeAg-negative CHB,HBsAg-negative phase have proportion 28.10%,25.00%,38.50%,8.20%,0.20%.1.2 Epidemiology character of patients with chronic hepatitis B virus infection: ratio and proportion of male patients in immune clearance and HBeAg-negative CHB is higher of those who in immune tolerance and inactive HBV carrier state (P<0.05),the female is contrary.Patients with chronic hepatitis B virus infection by vertical spreading infection in each natural history group is lower and lower,who has higher proportion in immune tolerance and immune clearance than inactive HBV carrier state and HBeAg-negative CHB (P<0.05),but patients by level spreading is reverse.1.3 Comparation of different gender in spreading manner: male and female group have ont obvious differences infected by vertical spreading or level spreading(χ2=2.753, P=0.097).1.4 Comparation of HBsAg level in different spreading manner of Patients with chronic hepatitis B virus infection: patients by vertical spreading have higher HBsAg level than those who by level spreading.2. Clinical data patients between 20~40 years old with chronic hepatitis B virus infection2.1 Correlational analyzing HBV DNA among HBV M ration,ALT of patients with chronic hepatitis B virus infection:HBV DNA has positive correlational dependence with HBsAg,HBeAg and ALT(r=0.222,P=0.000),(r=0.780,P=0.000),(r=0.303,P=0.000);HBsAg has positive correlational dependence with HBeAg(r=0.307,P=0.000).2.2 Comparation of HBV M ration on different level of HBV DNA: HBsAg level is gradually higher and higher; HBeAb level is gradually taper.But HBV DNA level is higher in HBeAg positive group than negative group(P<0.05).2.3 Epidemiology character of patients between 20~40 years old with chronic hepatitis B virus infection in ASC,CHB and LC group:it is correspond amout between male and female patients in ASC group,but from ASC group to CHB group to LC group,male patient is increasedly,and there is significant difference(χ2=33.01,23.95, 5.01,P=0.000,0.000,0.025),on the contrary,the female patient is not.There is no significant difference among those three group on spreading manner(χ2=0.42,0.08,0.00,P=0.515,0.780,0.968).2.4 In different clinical group,comparison among HBV DNA,HBV M quantitation and ALT:HBsAg level is gradually lower and lower from ASC group to LC group(P<0.05).HBeAg quantitation,HBV DNA level in CHB group are higher than those in group ASC and LC(P<0.05),but HBeAb quantitation is adverse(P<0.05).There is also significant difference among those three groups on ALT.2.5 Different infection pattern infects different expression of HBV DNA:there are nine infection modes in the total of 1000 case,and there is significant difference in HBV DNA between the dominating pattern"1,3,5"and"1,4,5"(t=8.81,P=0.000).[conclusion]1. Patients between 20~40 years old with chronic hepatitis B virus infection is in the majority at immune control phase and in the minority at HBsAg-negative phase.2. HBV DNA has positive correlational dependence with HBsAg,HBeAg and ALT; HBV DNA has negative correlational dependence with HBeAb;HBsAg has positive correlational dependence with HBeAg.3. The case in spreading manner infection is more in HbeAg positive group than that in negative group.4. Male and female group have ont obvious difference in spreading manner infection,all the people are impressionable of HBV.5. HBsAg level in different spreading manner of Patients with chronic hepatitis B virus infection: patients by vertical spreading infection have higher HBsAg level than that by level spreading.
