Objective To Study the effection of computed tomography(CT) and three-dimensional computed tomography reconstruction(3D-CT) in diagnosis and treatment of tibial plateau fractures Schatzker classification. Methods From January 2005 to February 2010 , there were 256 cases of tibial plateau fractures, that were examined preoperatively under X-ray, CT scans and 3D–CT.The examination results and Operative findings as the basis for Schatzker classification.To compare different standardization for the effect of Schatzker classification.Analyse the effect of CT scans and 3D–CT in Schatzker classification.All data using SPSS 17.0 for windows statistical software processing, and Chi-square test was used in the analysis of enumeration data, P<0.05 as statistically significant difference.Results Among 256 cases's tibial plateau fractures,there were 32 cases's diagnosis of X-ray is disagrees with CT, 9 tibial plateau fractures were not found on X-ray films, but on CT scans and 3D–CT the classification for 23 fractures were wrong.256 people were diagnosed by the check of CT scans and 3D–CT.The coincidence rate of X-ray was 87.5%. CT scans and 3D–CT has advantages in Classification Diagnosis of Schatzker classification (V-type),that can avoid missed diagnosis.Conclusions Plain CT scans and three-dimensional reconstruction could display the morphous of tibial plateau fractures completely and stereoscopically,it would be better for correctlly grouping of Schatzker of tibial plateau fractures.It could avoid especially missed diagnosis for tiny fissurate fractures,berm collapsed and posterior fractures of tibial plateau.
