Objective To study the clinical pathology, the expressions of p53, E-cadherin and ki-67, The survival and prognosis of preoperative TACE treatment to HCC patients after hepatectomy.To discuss the efficiency of preoperative TACE treatment to HCC patients Methods (1) 479 patients with resectable hepatocellular carcinoma and clinical pathological fom2003 to 2008 were divided into the group who underwent preoperative TACE and the group who did not. (2) The patients pathological specimens were treated by tissue microarray. The expressions of p53, E-cadherin and ki-67 were t examined by wo-step methods of PV-6000 of immunohistochemistry. Clinical pathological data, tumor recurrent rate and survival rate after hepatectomy were analyzed between the preoperative TACE group and non-TACE group. (3) The clinical pathological and the expressions of p53, ki-67 and E-cadherin between the preoperative TACE group and non-TACE group were analyzed by software SPSS 13.0,χ2-test.The tumor recurrent rate and survival rate between the preoperative TACE group and non-TACE group were analyzed by software SPSS13.0 Kaplan-meier, Log Rank. Results (1) There was a significantly difference in the tumor size between the preoperative TACE group and non-TACE group, other clinical pathological factors were no difference.. The tumor size in the preoperative TACE group (22/27) was larger than no TACE group (118/255) (χ2=10.738, P=0.001). Although the patients with high tumor pathological grade in the preoperative TACE group was only one (3.7%), there was no difference between the two groups tumor pathological grade (χ2=0.017, P=0.895)。(2) Positive staining of P53,Ki67were mainly located in the cell nucleus. The positive expression rate of P53,Ki67 in the preoperative TACE group was 25.9%(7/27) and 70.4%(19/27). The positive expression rate of P53,Ki67 in the non-TACE TACE group was 26.4%(66/250) and 72.9%(186/255). Positive staining of E-cadherin was mainly located in the cell nucleus and cytoplasm. The positive expression rate of E-cadherin in the preoperative TACE group was63.0%(17/27),and the non-TACE TACE group was57.1%(144/252). (3)The one year disease free survival rate in the preoperative TACE group was25.9%(7/27),and the non-TACE TACE group was30.9%(79/255).There was no difference between the two groups.Also,the two groups survival rates were no difference (χ2=0.298,P=0.585).Conclusions (1)The two groups clinical pathology,tumor pathological grade wre no different examed by the SPSS13.0,χ2-test. The preoperative TACE treatment has no adverse effect to the HCC patient's tumor pathological grade. (2)The expression of P53,Ki67 and E-cadherin has no difference between the preoperative TACE group and non-TACE group.So the preoperative TACE treatment has no adverse effect to the HCC tumor's Invasion, proliferation and metastasis. (2)The HCC patient with the preoperative TACE treatment can have the similar survival date with the non-TACE patient.The preoperative TACE treatment can improve the rate of tumor resection and it is a safe and effective treatment.
