| Background:Choroidal neovascularization caused by macular degeneration is a group of disease diffcult to treat at present.Hemorrhage,exudation,retinal detachment caused by CNV and scar formation at last were the main cause of blindness in CNV diseases including wet age-related macular degeneration, central exudative chorioretinopathy,pathologic myopia.Pathogenesis and treatment for the CNV study is the hot spots domestic and abroad.Photodynamic therapy (PDT) is a new and effective means to treat this kind of diseases and it has been already gradually spreaded and used.It is considered that the mechanism of PDT used for treatment of CNV was that the photosensitizer which injected in the body was specially combined with neovascular endothelial cells and more stagnated in the lumens of CNV.The photochemical reaction occurred after exposure in a definite wave length laser.The endothelial cells of CNV were then damaged and the lumens of CNV were blocked.The leakage of blood constituent stopped or decreased and the more damage of photoreceptor cells by CNV was controlled.But PDT can not exterminate the recur of the CNV,and it also has the defects like the inthorough treatment of the CNV.But CNV in some people recurrent in a very short period.The long-term Many domestic and international research shows that in the short term to maintain or slow the deterioration of eyesight has some advantages.But the PDT can not fundamentally prevent the occurrence of CNV,and therefore there is not complete on the treatment of CNV,not so long completely disappeared,easy recurrence.Maculopathy is related to spleen closely in traditional chinese medicine.Deficiency of spleen-Qi and its dysfunction of transformation and transportation are the pathology foundation of maculopaty.Water,blood stasis and phlegm are the pathology changes.Therefore invigorating spleen-Qi is the rule of treating Ben,while removing blood stasis,resolving phlegm and so on are of Biao.TCM is an objective physical phenomenon of life,is the individual life course,in the congenital and acquired genetic basis,showing the morphological structure,physiological function and psychological status such as an integrated,relatively stable characteristics.This characteristic determines the body's susceptibility to certain pathogens and pathologic changes in the type of bias.Treatment of Distinction physical factors with significant correlation with diagnosis and treatment of diseases of great significance.Instructors has used PDT and Jianpihuazhuofang to treat CNV for many years, all previous studies are focused on short-term observation of curative effect three months after treatment,the lack of long-term observation of curative effect.Purpose:1.Observe the long-term clinical efficacy of PDT and PDT combined with traditional Chinese medicine to treat choroidal neovascular diseases.2.Choroidal neovascularization observed in patients with type distribution of physical medicine, physical type and disease incidence and prognosis in order to find out some rules for the prevention and treatment of CNV disease to provide experience.Method:1. Paid a return visit to the CNV patients that once accepted PDT and PDT combined traditional Chinese treatment in our hospital from May 2005 to February 2009,this follow-up observation of 80 cases were collected 88,which occurred in age-related macular degeneration 33 eyes of 27 cases,17 cases of pathologic myopia in 18 eyes,central exudative chorioretinopathy 36 cases of 37. Longest follow-up time was 56 months,minimum 12 months.After their first PDT are taking spleen Huazhuo side divided into two groups of patients,PDT alone group and the Chinese intervention group.Detailed records from the disease since the patients were followed up for disease development and treatment,and measuring best corrected visual acuity,dilated fundus inspection,combined with previous imaging data,according to visual acuity,fundus evaluation of its long-term effects.2.According to 2009 implementation of the "Chinese Medical Association standards-Chinese Constitution Classification and Judging" (ZYYXH/T157-2009) to develop physical survey,each patient completed follow-up,"Chinese constitution classification and decision tables",calculation and transformation of the original sub- points determined by the standard body type.3.Statistical analysis using SPSS16.0 software. The above data analysis,we get the results.Result:1.After the initial treatment,38 eyes,recurrent PDT group,34.21% for 13 eyes,recurrence interval is the shortest 3 months,the longest of 26 months,average 9.9 months.Chinese intervention group 44,relapse 6,the recurrence rate was 13.64%,the recurrence interval minimum of 6 months,up to 15 months,an average of 10.8 months.2. In vision improvement and stable aspect, PDT group and Chinese medicine intervention group non-statistics difference; Three kind of diseases compare have the difference:The CEC patient prognosis is best, next is the PM patient, is the AMD patient once more.3.TCM Constitution survey: 80 example CNV patient physique distributed difference is remarkable, has statistics significance.Various physique type accounts for the proportion from high to low is:normal quality(27.5%),Qi deficiency(23.75%), Yang deficiency(18.75%) phlegm-wet quality(8.75%),wetness-heat(7.5%),blood-stasis(6.25%,Yin deficiency and Qi depression(both6.25%).Conclusion:Makes a follow-up visit from at present to this part of patient looked, the Chinese medicine intervention group's recurrence rate is lower than the pure PDT group relatively.In the vision improvement and the stable aspect,the CEC patient prognosis is best,next is the PM patient, is the AMD patient once more.The normal quality patient the index of stability is high after the treatment,the biased physique recurrence rate is higher than the gentle nature.
