Objective To observe the short-term and long-term clinical curative effect by taking method of clearing away heat, promoting blood circulation and arresting ulceration with enema decoction in treating ulcerative colitis (UC).Methods 143 patients with UC were involved from out-patient and in-patient departments of Jiangsu Provincial Hospital of Traditional Chinese Medicine.75 of them undertook therapy with enema of clearing away heat, promoting blood circulation and arresting ulceration (therapy group), while 68 others received therapy of Salicylazosulfapyridine(SASP) enema (control group), each with a course lasting for one year. Improvement of clinical symptoms and endoscopic manifestations as well as bioptic results were observed and compared, and recurrence after treatment of both groups were recorded. Results Clinical study showed that improvement of both main symptoms (as diarrhea, bloody stool with pus and abdominal pain) and secondary symptoms (as abdominal distention and burning sensation in anus) during active stage of UC (or in short term) in therapy group were better than those in control group, while some other symptoms like tenesmus, poor appetite and lassitude showed no difference in these two groups. During remission period (or in long term), after 12-month treatment,27 cases in therapy recurred while 30 cases in control recurred, showing a significant difference (P<0.05). Therapy group enjoyed a remission rate of 60.87%, and control with 40.00%, indicating better remission rate in therapy group compared with control group. Total effective rate and obvious effective rate in enteroscope in therapy group were relatively 89.58% and 68.75%, while the figure in control group were 55.00% and 37.50%, marking a significant difference (P<0.05). Total effective rate and obvious effective rate in mucosal tissue pathology in therapy group were relatively 81.25% and 60.42%, while the figure in control group were 50.00% and 40.00%, marking a significant difference (P<0.05).Conclusion It shows better curative efficacy in both short-term and long-term period to treat UC with method of clearing away heat, promoting blood circulation and arresting ulceration in enema decoction than Salicylazosulfapyridine(SASP) enema, especially in terms of improving TCM syndrome and colonoscopic changes and preserving remission.
