Clinical Studies With FMRI On Efficacy Of Acupuncture In Combination With Rehabilitation In Relieving The Ankle Dysfunction Of The Patients With Ischemic Stroke

Objective:1st,to observe the clinical effects of acupuncture in combination with rehabilitation in relieving the ankle dysfunction of patients with stroke for finding the most effective method that could improve ankle dysfunction after ischemic stroke.2nd,analyze the relationship between function of ankle and anatomy position of brain by functional magnetic resonance imaging.Method:In total of 38 patients of hemiparesis caused by ischemic stroke were randomly divided into two groups. Treatment group was given acupuncture with rehabilitation training, while the control group was treated by rehabilitation training only. Patients of the two groups were both given the support conventional symptomatic treatment of neurology. The course of treatment was 20 days,5 times per week. Both before and after the course of treatment, take the FMA and Bathel ADL index of lower limbs. Comparing and analyze the results of the two groups after treatments.At the same time, study the relationship between function of ankle and anatomy position of brain using functional magnetic resonance imaging in 6 patients of treatment group.Result:1st, The result of the treatment group was better than control group. The FMA of lower limbs improved. Each group had a significant difference(P<0.01)both before and after the course of treatment. And compared between the two groups,the difference was significant (P＜0.01).2nd, The result of the treatment group was better than control group. The Bathel ADL index of lower limbs improved. Each group had a significant difference(P<0.01)both before and after the course of treatment.The Bathel ADL index of lower limbs improved. And compared between the two groups,the difference was significant (P<0.05).3rd,treatment after a week of first patient,with the dorsiflexion and plantar flexion of sick ankel,primary motor cortex BA4(L,R),pre-motor cortex BA6(L,R) was activated,while after a month, primary motor cortex BA4(L),pre-motor cortex BA6(L,R) was activated.Compared the other four patients, the dorsiflexion and plantar flexion of nomal ankle activated the contralateral primary motor cortex (BA4),pre-motor cortex (BA6), the dorsiflexion and plantar flexion of sick ankle activated contralateral primary motor cortex (BA4),pre-motor cortex (BA6),ipsilateral primary motor cortex (BA4) and(or) pre-motor cortex (BA6).Conclusion:Acupuncture combined with rehabilitation therapy can relieve the ankle dysfunction of patients with stroke more effective than only rehabilitation therapy,which may improve blood circulation and cellular metabolism of movement-related area of brain.
