Objective: Cerebral infarction(CI), is the generic name of cerebral ischemic stroke, including cerebral thrombosis, lacutypingar infarction and cerebral embolism, etc. About 59.2%~85% of all stroke, cerebral bloodsupply is caused obstacles ischemia, hypoxia, resulting in limitations of ischemia braian tissue necrosis or encephalomlacia. As clinical common and frequently-occurring disease. In the convaleescence stage of this disease without specific effective treatment. According to clinical manifestations of this disease it belongs to the category of stroke. As early as in the Huang Di Nei Jing, there is awareness of stroke. After a long period of clinical practice experience summed up a wealth of syndrome differentiation. The correct rule on the differentiation is premise of treatment. The differentiation of symptoms of TCM is based on syndrome identification and then combined into syndrome from the symptom factors. It mast have major symptom factors in syndrome. It is only to seize the principal syndrome can you to seize the substance of the disease. Cerebral infarction at different times, different types will have different pathological changes. We conclude the length of the respective Traditional and Western medicine, combined clinical staging classification of cerebral infarction with Traditional Chinese Medicine Syndrome Differentiation of the principal syndrome. It is profit to improve the level of TCM Syndrome Differentiation. The purpose of this study is to approach the correlation between principal syndrome differentiation of TCM with Staging and Typing of the convaleescence stage of anterior circulation infarction. Started the differentiation combin Staging and Typing with the principal syndrome of the convaleescence stage of anterior circulation infarction. Our purpose is to provide new ideas and methods for the differentiation of cerebral infarction in TCM and to provide theoretical basis for the treatment in TCM. It is beneficial to guide stroke comprehensive treatment combining traditional and western medicine, and to improve cerebral infarction. The end result is to reduce mortality and disability. Methods: The total 390 cases of CI are chosen from inpatient in the Second Hospital of Hebei Medicine University Department of neurology. From September 2008 to January 2010. We divided the convalescence stage of CI into three stages: earlier convalescence stage, intermediate convalescence stage and later convalescence stage. And according to the severity threetypes: mild type, moderate typeand severe type. and severe type. At the same time we collect datas by using a method of comprehensive analysis by the four examination methods(a Chinese WangWen wen qie), and then use ZhuWenfeng dialectical symptom scale to dialectical. The symptom factors of disease in his research has thirty-three and the symptom factors of disease location has twenty. Based on the phased sub-type and the results of dialectical view to ascertain the symptoms of each type. Then use the principal factor analysis to extract the symptom factors analyzing there levance between Syndrome Differentiation of TCM with Staging and Typing of Cerebral Infarction.Result:1 The symptom factors of convaleescence stage of anterior circulation infarction is: The symptom factors of disease location in the mild of earlier convaleescence stage which in higher frequency are: meridian, liver, kidney, large intestine. The symptom factors of disease nature which in higher frequency are: phlegm syndrome, syndrome of fire-heat, yin asthenia, blood stagnationand, endogenous wind, blood deficiency, deficiency of vital energy, damp pathogen, predominant yang. The symptom factors of disease location in the moderate and severe type of earlier convaleescence stage which in higher frequency are: meridian, liver, large intestine, kidney, heart-spirit. The symptom factors of disease nature which in higher frequency are: phlegm syndrome, blood stagnationand, syndrome of fire-heat, yin asthenia, deficiency of vital energy, endogenous wind, damp pathogen, excess syndrome of stroke, blood deficiency, predominant yang. The symptom factors of disease location in the mild of moderate and later convaleescence stage which in higher frequency are: meridian, liver, large intestine, kidney. The symptom factors of disease nature which in higher frequency are: phlegm syndrome, blood stagnationand , deficiency of vital energy, yin asthenia, blood deficiency, syndrome of fire-heat, damp pathogen, predominant yang, endogenous wind. The symptom factors of disease location in the moderate and severe type of moderate and later convaleescence stage which in higher frequency are: meridian, kidney, large intestine, liver; the symptom factors of disease nature which in higher frequency are: phlegm syndrome, blood stagnationand , deficiency of vital energy, syndrome of fire-heat, yin asthenia, excess syndrome of stroke, damp pathogen, endogenous wind, predominant yang , blood deficiency.2 The major symptom factors of convaleescence stage of anterior circulation infarction is: The Principal factor 1 of the mild of earlier convaleescence stage are: meridian, blood stagnationand, phlegm syndrome; the Principal factor 2 is: deficiency of vital energy; the Principal factor 3 is: yin asthenia; the Principal factor 4 is: liver; the Principal factor 5 is: endogenous wind. The Principal factor 1 of the moderate and severe type of earlier convaleescence stage are: meridian, blood stagnationand, phlegm syndrome, excess syndrome of stroke; the Principal factor 2 is: deficiency of vital energy; the Principal factor 3 are: yin asthenia, liver; the Principal factor 4 is: heart-spirit; the Principal factor 5 is: large intestine. The Principal factor 1 of the mild of moderate and later convaleescence stage are: meridian, phlegm syndrome; the Principal factor 2 are: deficiency of vital energy, yin asthenia; the Principal factor 3 are: blood stagnationand, liver; the Principal factor 4 is: kidney; the Principal factor 5 is: large intestine. The Principal factor 1 of the moderate and severe type of moderate and later convaleescence stage are: meridian, phlegm syndrome, blood stagnationand; the Principal factor 2 are: deficiency of vital energy, yin asthenia; the Principal factor 3 are: heart-spirit; excess syndrome of stroke; the Principal factor 4 is: kidney; the Principal factor 5 is: large intestine.3 According to the major symptom factors for the principal syndrome is: the principal syndrome of the mild of earlier convaleescence stage are: obstruction of collateral caused by phegm and stagnationand, deficiency of both vital energy and yin. The accompaniment symptom is Endogenous liver wind. The principal syndrome of the moderate and severe type of earlier convaleescence stage are: obstruction of collateral caused by phegm and stagnationand, deficiency of both vital energy and yin. The accompaniment symptom is excess symdrome of stroke. The principal syndrome of the mild of moderate and later convaleescence stage are: obstruction of collateral caused by phegm and stagnationand, deficiency of both vital energy and yin. The principal syndrome of the moderate and severe type of moderate and later convaleescence stage are: obstruction of collateral caused by phegm and stagnationand, deficiency of both vital energy and yin. The accompaniment symptom is excess symdrome of stroke. Conclusion: The common principal syndrome of convaleescence stage of anterior circulation infarction isobstruction of collateral caused by phegm and stagnationand, deficiency of both vital energy and yin. This reflects the common internal rule in convaleescence stage of this disease. And the accompanied syndrome reflect that different staging and typing pathological change of TCM. Different staging and typing has a very close relationship between the principal syndrome in CI.To study the principal syndrome can open up new ideas for Chinese medicine treatment of cerebral infarction. It can be an important supplement to Chinese medicine treatment of cerebral infarction.
