Partâ… The Random Control Study of Implanting Sustained Release 5-fluorouracil about Safety During Operation in Patients with Esophageal Cancer and gastric cardiac adenocarcinomaObjective: :To evaluate the effieieney and safety of Implanting Sustained Release 5-fluorouracil during local chemotherapy with operation in combined therapy of Esophageal Cancer and gastric cardiac adenocarcinoma.Methods: All of the trial patients were randomly divided into two group,i.e.control group and experimental group. One group have 45 patients averagely. Sustained release 5-fluorouracil were implanted in 45 cases with esophageal cancer and gastric cardiac adenocarcinoma during the radical operation. While 45 patients as control group take radical operation merely. The liver and kindney functions, blood cell counts levels were measured on the 1st day of pre-operation and the 7th day of Post-operation respectively in all patients and the incidence of complications later two weeks were compared. Furthermore,to compare whether there was obviously differentment between the two groups that on the peri-operative changements of the incident information of toxicity of chemotherpy,the time of Pulling out thorax closed drainage tube,and the rate of mortality .Results: There were no significan differences of liver and kindney functions and blood cell counts between the control group and the experimental group.The time of pulling out thoracic close drainage tube in experimental group was longer than in control group (P<0.05) and the volume of chest drainage liquid in postoperative in experimental group was more than in control group,which did not delay rehabilitation.The incidence rate of infection of incisional wound in experimental group was 8.8% and 2.2% in control group(P<0.05).The incidence rate of the rest postoperative complications was no significan differences. 1 Patients arose mild nausea symptom and 2 Patients arose mild diarrhoea symptom in experimental group while 1 Patients arose mild nausea symptom and 1 Patients arose mild diarrhoea symptom in control group emerely during second week after operation.The others did not arise any toxicity of chemotherapy in both groups.Conclusion: Implanting Sustained Release 5-fluorouracil during operation which is recommended as clinical dose is safe for Esophageal Cancer,gastric cardiac adenocarcinomPartâ…¡The initial animal experiment study of 5-fluorouracil implanting drug to local tissue damageObjective: to initial investigate the possible harmful effects to local normal tissue of 5-fluorouracil implanting drug on operation passing animal experiment study.Methods: 20 trial rabbit were randomly divided into 4 group,one group 5.one of groups is control group. They were enforced the discission of air tube esophagus and cervical part AtV. The rest as experimental group were imbeded 5-fluorouracil implanting drug during operation. According to human body dose we can infer the dose of trial rabbit.They were divided low-dose group high-dose group super-high-dose group. After Operation to observe control and experiment groups as well as experiment group in different drug concentration gradient whether or no difference in part and systematic reaction aspect caused by chemotherapeutics(including infection of incisional wound activity amount food-intake bowel complaint and so on).After 3th,15th in implanting drug naked eye review the change of local tissu and draw the materials from animals to make patho-slice and to observe below the light microscope through aetio-operative approach. After 15th control group low dose group and high dose group were to put to death.The super-high-dose group was kept on observation. After 45th naked eye review the change of local tissu and draw the materials to make patho-slice and to observe below the light microscope through aetio-operative approach. experiment to end execute all of the trial animal.Results: There were significan differences in incidence rate of infection of incisional wound between the control group and the experimental group. No experimental group and control group differences in the incidence of diarrhea.Implantation of sustained-release agent 5-FU during operation,the effect to activity and food intake of low-dose and high-dose group experimental rabbit is less no significant difference between the two groups, but unlike the control group is more active. Compared with the control group and low dose, high-dose group,super-high-dose group had the most significant differences. The mortality rate of super-high-dose group increased significantly. Compared with control group there is significant difference (P <0.05).Super-high-dose group throughout the experiment were poisoned three experimental rabbits. Only the first death of experimental rabbits died on the third day after implantation of drugs, autopsy revealed partial necrosis of the trachea, a large number of mucus plug within the lumen, should be choked to death; second only to death of experimental rabbits died after the first drawn the third day, accompanied by the formation of localized skin infections fester, showing ulceration depth of the trachea, pathological anatomic cause of death for the artery wall rupture, arteriovenous fistula formation; third test rabbit died only the death of the second drawn 13days after , autopsy revealed fistula formation.Conclusion: Given the recommended dose range of application,the safety of 5-fluorouracil should be, but the high local concentration of local organizations, will also increase the negative impact of increased risk.
