Objective: Orexin is the neuropeptide secreted by the hypothalamus,which plays an important role in the central nervous system . It has extensive function in the aspects of regulating to diet, energy metabolism balance, sleep period, blood pressure wait, especially in the processes of increasing the food intake, sugar metabolism. Orexin is an important factor in regulating food intake, energy metabolism, and it has a regulatory role in intrauterine fetal growth and development,and then influence the growth of neonate,child a period,and puberty.The abroad once reported that, the neonate has higher orexin level compare with child. The Leptin is a kind of protein hormone which is mainly secreted by white fat tissue,and have the opposite functin with orexin.And it can repress food intake and increase organism total energy consumption. Some scholars have found that Orexin neurons in Lateral hypothalamus and fornix contain leptin receptors and suggested that they may have interanctie. Adiponectin is a kind of protein hormone discovered in the recent ten years ,which is secreted by the fat cell.It extensively participates in energy metabolism , especially in fat metabolism and sugar metabolism, relating to the development of obesity,artherosclerosis,diabetes.At present ,there have no report about the relationship between neonate umbilical artery's orexin level weight,bodylength,head circumference and BMI,and also no report about the relationsip between orexin and leptin., between orexin and Adiponectin in the growth of the neonate.The study is to measure the levels of orexin,leptin and adiponectin in different gestational age neonates,and to study the relationship between the orexin and the birth weigth,BMI,bodylength,head circumference,and the relation- ship between orexin and leptin ,orexin and adiponectin , to further explore the interranctions of factors affecting neonatal growth and development .Methods: From 2009,6 to 2009,10,90 healthy neonates of Hebei Medical University were selected,and were devided into two groups according to the diagnostic standard of the sixth publishied pediatrics about the categories of neonate:the fullterm infant and the preterm infant(the 80 fullterm infant, the 10 preterm infant).The fullterm infants inclued 37 males and 43 females,gestational age37-41 ( 39.52±0.84 ) W, bornweight 2850-4900(3420±350)g,length 48-53.5(51.45±1.41)cm,head circumference 31-36(33.59±1.54)cm;The fullterm infants inclued 6 males and 4 females, gestational age 34+1-36+6(35.61±0.9)W, bornweight 1.978-3.8(2.64±0.62)kg,length 46.5-52(49.05±2.25)cm,head circumference 29-35(32.2±1.46)cm;The umbilical artery blood 7 ml were collected after the neonate expulsion and before the placenta expulsion and were centrifugalized for 10 minutes and 3000r/min rotational speed,and then serum 2ml were taken out and were stroed at -80℃until analyzsis. The newborns gender,birth weight,body length,head circumference and BMI[BMI= birth weight (kg) / body length (m)2], were strictly recored and orexin,leptin and adiponectin were measured by euzymelinked immunosorbent assay。Results:1 The result of orexin level in the neonate's umbilical artery serum:1.1 The result of orexin level in the fullterm infants The result of orexin level in the fullterm infants was 7.40±1.29ng/ml,the result of orexin level 7.26±1.11ng/ml in the female fullterm infants and 7.58±1.49ng/ml in the male fullterm infant,the significant differenc(eP<0.05)were shown.There existed negative correlation betwween the orexin level and weight(r=-0.384, P<0.05),headcircumference (r=-0.345, P<0.05),length (r=-0.294, P<0.05),and BMI(r=-0.309, P<0.05)。1.2 The result of orexin level in the preterm infantThe result of orexin level in the preterm infant was 10.61±2.32ng/ml,and weight was 2.64±0.62kg. There negative correlation betwween the orexin level and weight(r=-294, P<0.05),bodylength(r=-0.801, P<0.05), headcircumference (r=-0.863, P<0.05) and BMI(r=-0.772, P<0.05).1.3 The result of orexin level in the fullterm infant was 7.40±1.29ng/ml,and 10.61±2.32ng/ml in the preterm infant , they had significant difference(P<0.05).2 The measurement of leptin level in the neonate's umbilical artery serum2.1 The result of leptin level in the fullterm infantThe result of leptin level in the fullterm infant was 6.56±1.38 ng/ml,The leptin level and weight have positive related(r=0.436,P<0.05), The leptin level and head circumference have positive related(r=0.436,P<0.05),The leptin level and length have positive related(r=0.568,P<0.05),The leptin level and BMI have positive related(r=0.439,P<0.05). The positive correlation is revaled between the leptin level and the weight(r=0.436,P<0.05),bodylength(r=0.568, P<0.05),head circumference (r=0.436,P<0.05)and BMI (r=0.439, P<0.05)in the fullterm infant.2.2 The result of leptin level in the preterm infant The result of leptin level in the preterm infant was 6.46±1.21ng/ml,The leptin level and weight have positive related(r=0.65,P<0.05), The leptin level and head circumference have positive related(r =0.69,P<0.05),The leptin level and length have positive related(r=0.83,P<0.05),The leptin level and BMI have positive related(r=0.547,P<0.05).2.3 The result of leptin level in the fullterm infant was 6.56±1.38 ng/ml,The result of orexin level in the preterm infant was 6.46±1.21ng/ml,they had significant difference(p<0.05).3 The measurement of adiponectin level in the neonate's umbilical artery serum3.1 The result of adiponectin level in the fullterm infantThe result of adiponectin level in the fullterm infant was 14.05±3.32μg/ml, The positive correlation is revaled between the adiponectin level and the weight(r=0.484,P<0.05),bodylength(r=0.294, P<0.05),head circumference (r=0.531,P<0.05)and BMI (r=0.482, P<0.05)in the fullterm infant. 3.2 The result of adiponectin level in the preterm infantThe result of adiponectin level in the preterm infant was 11.84±1.86μg/ml,the adiponectin level and weight present positive related(r=0.378 P<0.05), the adiponectin level and head circumference have positive related(r=0.471 P<0.05),the adiponectin level and length have positive related(r=0.572 P<0.05),the adiponectin level and BMI have positive related(r=0.286 P<0.05).3.3 The result of adiponectin level in the fullterm infant is 14.05±3.32μg/ml,the result of orexin level in the preterm infant is11.84±1.86μg/ml,they have sinificant difference(p<0.05).4 The relationship between the leptin level and orexin level The levels of orexin and leptin of the umbilical artery serum had no obvious relativity in the fullterm infant (r=-0.157 ,P=0.083), The levels of orexin and leptin of the umbilical artery serum had no obvious relativity in the preterm infant(r=-0.5,P=0.07)。5 The relationship between the orexin level and adiponectin level The levels of orexin and adiponectin of the umbilical artery serum had negative relativity in the fullterm infant (r=-0.357, P<0.05),The levels orexin and adiponectin of the umbilical artery serum have negative relativity in the preterm infant(r=-0.531, P<0.05)。Conclusions:1 As the gestational age increase, orexin level comes to lower, leptin and adiponectin come to to higher.2 Between fullterm infant and the preterm., The orexin and leptin have no obvious relativity, The orexin and adiponectin levels have the negative relativity.3 There existed negative correlations betwween the orexin level and weight,headcircumference,length,and BMI in the fullterm infant and the preterm infant.
