Objective: The carcinoma of larynx is one of the malignant neoplasm in otolaryngology and head and neck surgery. It is related to varied genetic expression which caused by a deal of factors as same as other kinds of carcinomas. Signaling by TGFβ1 regulates cell proliferation, differentiation, migration and apoptosis. Signaling by TGFβ1 to nucleus is mediated by typeâ… receptor (TβRâ… ) and typeâ…¡receptor (TβRâ…¡) and the intracellular signal transducers, the Smads (R-Smads and CO-Smads), but Smad7 as an I-Smad inhibits signaling mediated by TGFβ1. Signaling disorder promted malignant cells development and progression. Transforming growth factorβ1( TGFβ1) is a cytokine with double regulator in Signaling. TGFβ1 is known as a tumor suppressor in early stage of tumorigenesis because it inhibit proliferaty of tumour cell and make signal transduction pathway of tumour cell inactive; In advaced tumor, TGFβ1 act as a tumor promoter, it can offer a suitable environment for tumour to develop, infotrate and metastasize, through stimulating angiogenesis or immune suppression and forming extracelluar matrix. We regard laryngeal squamous cell carcinoma as study object, to investigate the expression of TGFβ1, TβRâ… , TβRâ…¡and Smad7 in laryngeal squamous cell carcinoma and adjacent non-neoplastic tissues by Immunohistochemistry, to explore their role in the process of tumorigenesis, progression and metastasis of laryngeal squamous cell carcinoma and their correlation.Methods:1 Immunohistochemistry for TGFβ1, TβRâ… , TβRâ…¡and Smad7 expression was performed on cryostat section from 43 laryngeal squamous cell carcinoma tissues and 12 adjacent non-neoplastic tissues.2 SPSS 13.0 was applied to analyze the results of experiment. P≤0.05 indicating significant difference. P≤0.01 indicating very significant difference.Results:1 Expression of TGFβ1 and relation with clinical parametersThe expression of TGFβ1 in laryngeal squamous cell carcinoma tissues and adjacent non-neoplastic tissues was detected, there is statistic significance between them (P<0.01). The positive expression rate of TGFβ1 protein in laryngeal squamous cell carcinoma tissues was 74.42%, while that in adjacent non-neoplastic tissues was 25.00%, the expression of TGFβ1 is stronger than that in adjacent non-neoplastic tissues. Expression of TGFβ1 protein was correlated with clinical stages(the expression inâ…¢,â…£is stronger than that inâ… ,â…¡,p<0.05), lymph node metastasis(the expression in lymph node metastasis is stronger than that in no lymph node metastasis,p<0.01) and pathological grading of tumor (p<0.05), but that wasn't correlated with age, sex, clinical classification (p>0.05).2 Expression of TβRâ… and relation with clinical parametersThe expression of TβRâ… in laryngeal squamous cell carcinoma tissues and adjacent non-neoplastic tissues was detected, there is no statistic significance between them(P>0.05). though the expression of TβRâ… in laryngeal carcinoma is weaker than those in adjacent non-neoplastic tissues. Exression of TβRâ…¡protein was correlated with clinical stages(the expression inâ…¢,â…£is lower than that inâ… ,â…¡, p<0.05), lymph node metastasis(the expression in lymph node metastasis is lower than that in no lymph node metastasis, p<0.05), but that wasn't correlated with age, sex, clinical classification and pathological grading of tumor (p>0.05).4 Expression of Smad7 and relation with clinical parametersThe expression of Smad7 in laryngeal squamous cell carcinoma tissues and adjacent non-neoplastic tissues was detected by immunohistochemistry S-P, there is statistic significance between them(P<0.01). The positive expression rate of Smad7 protein in laryngeal squamous cell carcinoma tissues was 76.74%, while that in adjacent non-neoplastic tissues was 8.33%,indicating the expression of Smad7 in laryngeal carcinoma is stronger than that in adjacent non-neoplastic tissues. Expression of Smad7 protein was correlated with clinical stages(the expression inâ…¢,â…£is stronger than that inâ… ,â…¡, p<0.01), lymph node metastasis(the expression in lymph node metastasis is stronger than that in no lymph node metastasis, p<0.05) and pathological grading of tumor (p<0.01), but that wasn't correlated with age, sex, clinical classification (p>0.05).5 Correlations among positive expression of TGFβ1 and TβRâ… , TGFβ1 and TβRâ…¡, TGFβ1 and Smad7The positive expression rate of TGFβ1 of laryngeal squamous cell carcinoma tissues was 74.42%, that of TβRâ… is 32.56%, that of TβRâ…¡is 39.53%, and that of Smad7 is 76.74%. There is a negative correlation between the expression of TGFβ1 and TβRâ… , TGFβ1 and TβRâ…¡(r=-0.503,P<0.01; r=-0.398,P<0.01). but a positive correlation is in the of expression of TGFβ1 and Smad7(r=0.434,P<0.01).Conclusions:1 TGFβ1, TβRâ… , TβRâ…¡and Smad7 protein all express in laryngeal squamous cell carcinoma tissues and adjacent non-neoplastic tissues.2 The qualitative expression of TGFβ1 and Smad7 in laryngeal squamous cell carcinoma was higher than that in adjacent non-neoplastic tissues. But the qualitative expression of TβRâ… and TβRâ…¡protein in laryngeal squamous cell carcinoma was lower that in adjacent non-neoplastic tissues. These were correlated with tumorigenesis of larynx.3 The expression of TGFβ1, TβRâ… , TβRâ…¡and Smad7 protein was correlated with clinical stages, lymph node metastasis. That of TGFβ1 and Smad7 protein was correlated with pathological grading of tumor. They may contribute to progression and metastasis of laryngeal squamous cell carcinoma.4 There is a correlation between the expression of TGFβ1 and TβRâ… , TGFβ1 and TβRâ…¡, TGFβ1 and Smad7. The four factors ineracting may contribute to tumorigenesis, progression and metastasis of laryngeal squamous cell carcinoma.
