The Anlysis Of The Death Of Subarachnoid Haemorrhage

Objective:In order to provide experience to the treatment of subarachno- id haemorrhage,and to make clear of the clinical exactly features of the relative complications,we need to know the reasons of the death with the patients in hospital of subarachnoid haemorrhage and to know what we need to learn from these cases.Methods: By rearrangementing the clinical data of the patients after sub- arachnoid haemorrhage who had been treated in hospital between Oct. 2009 and Feb.2010,we selected 16 cases with the end result of death,and analysed the primary cause of mortality in SAH patients and discussed the preventive and therapeutic treatments what we need to take.Discussion:The major causes and preventions of the death after subarac- hnoid haemorrhage include 1.rebleeding,cerebral hernia,the prevention include removing inducement,finding out the reason of the bleeding in time, surgical clipping or endovascular coiling should be performed to reduce the rate of rebleeding in early phase,observing patient's conditions tightly in the duration of hospital stay,recheck cranial CT promptly,when necessary do emergency operation.2.cerebral vasospasm,preventions include triple-H therapy, calcium antagonists, vasodilator agents, getting rid of the blood in the subarachnoid cavity as early as possible.3. epilepsy,preventions include curing intracal pathological changes to get rid of inducement of epilepsy,observing the breath intimately.4.complications,patients in come should prevent pulmonary infec- tion and alimentary tract hemorrhage,keep electrolyte in balance,control blood glucose in normal level.for SAH patients choosing the time of opration is also in great significance.Conclusion:The primary causes of death in SAH patients were not exactly the same before and after operation.The major cause of death of the patients before operation was rebleeding,cerebral hernia.The major cause of death of the patients after opration was postoperative complications,which includs postoperative intracranial hematoma, pulmonary infection, electrolyte disturbances,alimentary tract hemorrhage,long-term in a state of unconscious- ness.Postoperative complications may not fatal,but could aggratate patient's conditions,increase mortality.So preventing and curing positively postoper- ative complications also could decrease mortality. Before or after operation severe cerebral vasospasm induced by subarachnoid haemorrhage can lead to die.
