Objective:To evaluate the outcome of two fixation methods for 55 cases Pilon fracture and put forward the best treatment method.Material and methods: From Jan. 2005 to Jan. 2009, 55 patients (43men and 12 women), divided into two groups according to the fixation methods: 26 patients were treated with ORIF (open reduction and internal fixation) ; 29 patients were treated with two- stage delayed ORIF.Results:1.All the 55 operations were performed by the same surgeon team in the same hospital。All cases classified into type—II and type-Ⅲaccor- ding to Ruedi-Allgower , and there were no significant difference among the two groups 0>0.05).2.After a mean follow-up of 13 months(range 6 to 24 months),it was found that there were significant differences among the two kinds of treatment methods.The group B got the better results'with82.8%good percentage comp- ared to 61.5%of group A respectively, according to Mazur's scoring system.Conclusion: The two—stage delayed open reduction and internal fixati- on may provide satisfactory fracture reduction and clinical results for severe Pilon fracture as decreasing the incidence of incision infection,delayed union and rigid joint,so it is considered that this method is the first method of treatment for Pilon fracture.
