Purpose: From the imaging fracture after reduction, early, and late complications of postoperative incidence, recent review limb ankle function, comparison of the clinical effects of internal fixation and limited internal fixation combined with external fixation for the treatment of Ruedi type III Pilon fracture, provide basis for the selection of operation type ruedi III pilon fractureMaterial and method:In 2012 January to 2013 January were collected in the Department of or thopaedics of intertrochanteric fracture treatment of Ruedi type III Shenyang Hospital Department of orthopedics of tibial Pilon, a total of 56.67 cases, including male 38 cases, female 22 cases; the distribution range in age between 20-57 years old, mean age 34.76 years; 40 patients with tibial Pilon fractures on the right side, left side patients in 20 cases. The cause of injury: 34 cases of traffic accident, falling injury in 14 cases, bruise in 8 cases, 4 cases of injury caused by other reasons. Admitted patients were closed fracture. Trauma to the operation reset interval time distribution of 7d-14 d, the average 10.84 d. According to the different uses hand type, divided into A, B two groups, namely, open reduction and internal fixation in the treatment group and the limited internal fixation combined with external fixation in treatment groupResults: After fracture reduction imaging showed no significant statistical difference; early postoperative complication rate was A, the B group is obviously different, among them, the A group the incidence rate was 26.67% in B group, the incidence rate was 6.67%, with statistical significance two. In the comparison of ankle joint function after operation, using the standard Mazur ankle joint scoring, recovery at 6 months postoperative examination two groups of patients with ankle joint function were excellent and good rate was 20%, A group, B group, the excellent and good rate was 26.67%, the excellent and good rate of B group was higher than that in group A, was not statistically significant (P>0.05); to restore the situation 12 months after operation, the excellent and good rate was 36.7% in A group, B group, the excellent and good rate was 70%; in the recovery 18 months after operation, the excellent and good rate was 46.67% in A group, B group, the excellent and good rate was 76.67%. To sum up, visible 12-18 months after operation between the satisfactory rate in the B group were significantly higher than the A group, there was singnificant(P<0.05).Conclusion:Combined with external fixation in the treatment of Pilon fractures of type in the early postoperative skin necrosis and infection incidence was significantly lower than the internal fixation method, and there is statistical significance, but no significant difference after reduction imaging changes of statistical data. At the same time, limited internal fixation combined with external fixation for the treatment of fractures in ankle joint function recovery after from 12 to 18 months after the operation has obvious advantage, with statistical significance.
