PURPOSE To discuss the clinical application and therapeutic strategy ofstandard large trauma craniotomy for contrecoup contusion andlaceration in frontal lobe.Methods Using clinical retrospective comparing study, 48 cases of contrecoupcontusion and laceration in frontal lobe in our hospital from May. 2007to Jun. 2009 were analysed retrospectively with their imaging feature:and 32 of them merged with intracerebral hematoma, 8 merged withdiffuse brain swelling and 11 merged with occipital extraduralhematoma in image foundings. All patients got their CT scan beforesurgery and reexamined after surgery in time, follow-up with Glasgowoutcome scale at 6 month, and compare the Statistical differencebetween three groups.Results In accordance with the GOS, 36 cases survived well, 8 suffered frommoderate disability, one lived in persistent vegetative state and onedead.Conclusion Contrecoup contusion and laceration in frontal lobe often merges withoccipital extradural hematoma and diffuse brain swelling. Earlydiagnosis and treatment to the delayed occipital extradural hematoma,and standard large trauma craniotomy in treatment of heavycontrecoup contusion and laceration with diffuse brain swelling infrontal lobe are the key measures to improve the rate of successrescue.
