Objectives: Investigate the clinical and endoscopy characteristics of Barrett's esophagus and the relationship between Barrett's esophagus, bile reflux and reflux esophagitis. And this study compared the conventional biopsies versus Lugol's–directed biopsies for the diagnosis of Barrett's esophagus and the detection of intestinal metaplasia and dysplasia. To evaluate the value of Lugol's staining in diagnosis of Barrett's esophagus, in order to provide reference for clinical doctors.Methods: 1. Retrospect analysis of 773 cases Barrett's esophagus in Peking University Shenzhen Hospital from 1999 to 2010. The age, sex, form in endoscopy, histology of biopsy and relationship between Barrett's esophagus, bile reflux and reflux esophagitis was analyzed.2. This is a retrospect study comparing conventional biopsies versus Lugol's–directed biopsies for the diagnosis of Barrett's esophagus, the detection of intestinal metaplasia and dysplasia in 64 patients with endoscopic suspected esophageal metaplasia. The age, sex, doctor, date, biopsies and form in endoscopy in two groups were matched.Results: 1. The detection rate of Barrett's esophagus in Peking University Shenzhen Hospital from 1999 to 2010 increased gradually, and the average detection rate was 0.82%. Most of Barrett's esophagus was middle-age and male versus female was 1.32:1. 89.32% of Barrett's esophagus patients were with short segment Barrett's esophagus and islands of Barrett's esophagus is the most form in endoscopy, next was tongue like. 8.80% of Barrett's esophagus patients were with reflux esophagitis, it was more than that non-Barrett's esophagus. 2. The sensitivity of Lugol's staining in diagnosing Barrett's esophagus was 81.25% (26/32), while the conventional endoscopy was 56.25%(18/32). The discordance between the two techniques was significant(X2=4.7, P<0.05), but not for detection of intestinal metaplasia and dysplasia.Conclusions: 1. The detection rate of Barrett's esophagus was increased year by year. Most of Barrett's esophagus patients were middle-age male.2. Most of Barrett's esophagus patients were with short segment Barrett's esophagus and islands form in endoscopy.3. There was close association between Barrett's esophagus and reflux esophagitis.4.The value of Lugol's staining in diagnosing Barrett's esophagus is superior to conventional endoscopy, but the value for detection of intestinal metaplasia and dysplasia need more samples to support.
