BackgroundChina is a high state of esophagus cancer, until now the exact cause is still not clear. Early detection, diagnose and treatment of the esophagus cancer particular the esophagus cancer ailments has very important significance in esophagus cancer prevention. In the development process of cancer, the cancer ailments is a active and instability in the stage of development, it has the bidirectional extension characteristics. Lugol's solution of esophagus membrane is a effective methods in the detection of.. How to detect the early pathological changes of the pathological, using the pathology and modern molecular technology timely identification early esophagus cancer, has important significance in the precaution, early diagnosis and it is the hot spot of clinical pathology research in the recent years.ObjectiveTo utilize the Lugol's solution combined with the biopsy tissue to detect the protein of COX-2, EGFR, ki-67,VEGF-C and P63. To analyze the inner connection among the image classification of endoscopic dye, the change of pathologic histology and the change of gene protein expression. To investigate the molecular mechanism and early elements events of the precancerous changes. Providing the new scientific basis in the disgnosis of esophagus cancer and precancerous changes.Methods1. Looking at the whole esophagus membrane under endoscope, using the Lugol's solution of 1.25 percentage, recording the chromatosis, section, borderline and extent of mucous membrane of esophagus, then to classified the images. The dyeing standards based on Masaki method, then classified the above records.2.Affixing biopsy tissue by using formalin of 10 percentage, HE dyeing to histology diagnosis. It was divided into low level and high level of intraepithelial tumor.3. From the 1879 cases,randomly select 64 cases of shallow cancer, 34 cases of high level intraepithelial tumor , 35 cases of low level of intraepithelial tumor and 13 cases with normal membrane. To detect the expression of protein COX-2, EGFR, VEGF-C, ki-67 and P63 by using the immunohistochemistry dyeing.4. To research the results of the dyeing classification of endoscope, the histology diagnosis and the expression results of gene protein by comparative methods and then making statistical analysis. Results1. According to the analyzed results, the image level of macroscopic observation by the Lugol's solution had very significantly negative correlations with the results of pathologic diagnosis.2. The results of immunohistochemistry dyeing displayed that all the constituent ratios had statistical differences among COX2, VEGF-C, GEFR, Ki67 and P63 of the pathology class at different levels. There had a positive correlations between pathological classification and the expression amount of the above genes. The expression amount of COX2, VEGF-C, GEFR, Ki67 and P63 rised along with the pathology class raised. All the expression amounts among VEGF-C, EGFR, Ki67 and P63, EGFR and Ki67, Ki67 and P63 had positive correlations.Conclusions1. The technology of the Lugol's solution assisited with endoscope and pathological examinations had become today' s best method of detecting and diagnosing the precancerous change of esophageal carcinoma, it was not only accurate and reliable but surmounted the arbitrary and blindness of endoscope examination. It played a most basic security role for the future research in precancerous change.2. In the early pathological changes of esophageal carcinoma, the positive express rate and express strength of Cox-2 raised, it cued that Cox-2 was an early elements event in esophagus cancer ailments. The protein expression maybe played an important role in the progress of epidermal cancer.3. There has a positive correlation between the express amount and the pathological classification of VEGF-C. It could not only be prompted the malignant differentiation degree, but used as an early warning indicators of esophagus cancer.4. The enhanced EGFR was an elements event in the progress of early esophagus cancer, it has an midrange and magnitude express in the biopsy structure. Predicting the pathological changed into the cancer ailments stage. It could be used as a biological indicators of the esophagus cancer ailments.7. The expression amount of COX2, VEGF-C, GEFR, Ki67 and P63 rised along with the pathology class raised. All the expression amounts among VEGF-C, EGFR, Ki67 and P63, EGFR and Ki67, Ki67 and P63 had positive correlations. That indicated that the irregular expressions of protein genes mentioned above had close relationship with the development of tumor. There were many factors, many genes synergies mechanism. The study shows the multiprotein genes combined testing had very important meaning in predicting the development and turnover of tumor from the different mechanisms. Particularly in the polarization trend of intraepithelial bump and identification of the innocence disease and malignant disease.
