| Background and objective:Chronic atrophic gastritis (CAG) is a common digestive disease, belongs to "Feeling of fullness", "Wei pi", "Stuffy Sensation of Deficiency Type", "Stomachache", "Noisy of stomache" and other areas in Traditional Chinese Medicine (TCM). The detection rate of CAG is higher than before with the application of gastroscopy. Intestinal metaplasia (IM) and intraepithelial neoplasia (IEN) are named Precancerous lesions of gastric cancer (PLGC). The research on PLGC has become hot spot of how to prevent gastric cancer (GC) in recent years due to it can appear canceration. The treatment of CAG’s modern medicine haven’t any effective therapy which mostly are symptomatic treatment and regular followed up, then operation if necessary, but it is difficult to reverse the development trend of PLGC. Meanwhile TCM play a unique advantage in the treatment of CAG. TCM not only can improve clinical symptoms, but also can prevent the presence of GC in PLGC. In this study, I will analysis the clinical information that collected through clinical, then patients who meet the inclusion criteria were treated by jianpihuayu detoxification method, to make an objective evaluation of its efficacy and discuss CAG possible pathogenesis and characteristics of the prescriptions, thus it can provide the theoretical basis for the clinical application.Method:According to the inclusion criteria of PLGC, the cases were divided into treatment group and control group.The treatment group has 51 patients treated by self jianpihuayu detoxification formula,2 times/day. There are 15 cases in control group treated by Weifuchun tablet,4 tablets/time,3 times/day. A course of treatment is 3 months, and the both of groups were treated by 2 courses. The two groups were not allowed to take other drugs during the study. Clinical symptoms, endoscopic features, pathology, Helicobacter pylori infection were observed before and after treatment, and statistical analysis were made to compare the treat effect.Results:CDThe correlation of PLGC with sex and age:age of oneset is mainly between 50-59 years and 60-69 years, the incidence of all ages and genders don’t have significant relationship (P<0.05). ②The situation of H.pylori infection:H.pylori infection rate was 68.8%, eradication rate was 82.7%. ③The predisposing factors of CAG mostly are poor diet (56%), climate change (47.7%) and emotional disorders (36.7%).④CAG’s incidence is closely related to hobbies such as hot food (61.5%), greasy food (33%) and smoke (32.1%). ⑤ After 2 courses, the total effective is 76.7% in the treatment group and the control group is 25%, the difference is significant (P<0.05). The effective rate of symptom scores is 100% in treatment group and the control group is 86.7%, the difference is significant (P <0.05). ⑥ After the treatment by jianpihuayu detoxification formula, glandular atrophy and dysplasia are significant improved (P<0.05), but there is not significant improvement in intestinal metaplasia (P>0.05). Glandular atrophy, intestinal metaplasia and dysplasia are not significant improved in the control group (P<0.05). In terms of dysplasia the treatment group is significantly improved compare to the control group (P<0.05), but both two groups are not significant difference in improving the gastric atrophy and intestinal metaplasia (P> 0.05).Conclusion:Jianpihuayu detoxification formula can effectively relieve the clinical symptoms of PLGC, reduce the degree of dysplasia and even reverse it to normal. |
