Research objective:1、To identify whether there is a difference in the presence of CD31+/CD42b in the plasma of endothelial microparticles in patients with IgA nephropathy and healthy people;2、To investigate the clinical significance of endothelial microparticles in patients with IgA nephropathy;3、To identify whether there are differences in plasma levels of endothelial microparticles in patients with IgA nephropathy and IgA nephropathy without hypertension;4、To investigate the risk factors of endothelial dysfunction in patients with IgA nephropathy;5、To investigate the correlation between TCM syndrome differentiation and endothelial microparticles in IgA nephropathy.Research methods:To select from 2015 October to 2016 March in Chinese Japan Friendship Hospital outpatient and ward, by renal puncture biopsy for definitive diagnosis of IgA nephropathy patients (32 cases, including IgA nephropathy with hypertension group (n= 15) and IgA nephropathy without hypertension group (17 cases), and the establishment of healthy people in the control group (29 cases), with the exception of other chronic diseases such as coronary heart disease, diabetes, cancer, heart failure, thrombotic diseases, rheumatic autoimmune diseases), records of patients with general information such as sex, age, nationality, family history, smoking history, drinking history, blood pressure, blood lipids, blood glucose, body mass index, time of onset, had been taking drugs, creatinine, uric acid, urine protein. After determining the experimental object, from each experimenter in peripheral blood extraction 3ml peripheral blood, then two centrifugal (first 16 degrees,1500g, 10min supernatant, and 4 DEG C,13000g, 10min), platelet poor plasma, into the 80 DEG thermostat cryopreservation, the specimen to be collected together, a freeze-thaw, adding specific monoclonal antibody CD31, CD42b, using flow cytometry to detect micro vesicles, and using SPSS statistical software for analysis.Research results:The number of CD31+/CD42b" in serum of patients with IgA nephropathy (312.50±248.250) was significantly higher than that in healthy group (98.31±73.81) P=0.000, (P<0.05) the difference was statistically significant. CD31+/CD42b+platelet microparticles count in IgA nephropathy group (3485.84±2634.75) was significantly higher than that in healthy group (1887.24±1876.752), P= 0.005 (P<0.05) the difference was statistically significant.32 cases of IgA nephropathy patients were classified as IgA nephropathy with hypertension (n= 15) and IgA nephropathy without hypertension (n= 17) in the two groups. Results show that IgA nephropathy with hypertension endothelial microparticles number (432.13±269.51, n= 15) significantly higher than IgA nephropathy patients without hypertension (206.94±174.90, n= 17), P= 0.008, P< (0.05), the difference has statistical significance; platelet microparticles of two groups, P= 0.710, P> 0.05, was no significant difference. According to the Spearman correlation analysis showed that, endothelial microparticles related with platelet microparticles, age,24 hours urine protein, RBC in urine, urine occult blood, creatinine, uric acid, urea nitrogen; IgA nephropathy patients with TCM syndrome types were deficiency of both qi and Yin (11 cases)> deficiency of spleen and kidney qi (8 cases)> spleen kidney yang deficiency (7 cases)> liver kidney yin deficiency syndrome (6 cases), TCM syndrome differentiation between groups endothelial microparticles number without significant difference.Research conclusion:1、The endothelial microparticles in IgA nephropathy patients were significantly higher than those in healthy people;2、Endothelial dysfunction may be present in patients with IgA nephropathy3、The number of endothelial microparticles in serum of IgA nephropathy patients with hypertension was significantly higher than that in patients with IgA nephropathy without hypertension, it showed that IgA nephropathy combined with hypertension may lead to more serious endothelial injury;4、Correlation between endothelial microparticles and platelet microparticles, age,24 hours urine protein, RBC in urine, urine occult blood, serum creatinine, blood uric acid, blood urea nitrogen;5、According to the type of syndrome differentiation of TCM, in patients with IgA nephropathy and TCM symptoms appear the order of frequency for the type of deficiency of both qi and Yin, yin deficiency of liver and kidney, Yang deficiency of spleen and kidney, spleen and kidney deficiency type. There was no significant correlation between the number of endothelial microparticles in each group.
