Correlation Between Peripheral Blood Endothelial Cell Subsets, Circulating Endothelial Cell Subsets And Endothelial Microparticles In Patients With Coronary Heart Disease And Blood Stasis Syndrome

Objective:To investigate the difference in the level of endothelial progenitor cell(EPC)subsets,circulating endothelial cell(CEC)subsets and endothelial microparticles(EMP)in peripheral blood of patients with blood stasis syndrome of coronary heart disease(CHD)and healthy people,flow cytometry was used to detect the three indexes above.And then we intended to gain the correlation between endothelial microparticles and endothelial progenitor cell subsets and circulating endothelial cell subsets.On the other hand,we intended to obtain the difference in the level of endothelial progenitor cell subsets,circulating endothelial cells subsets,and endothelial microparticles in patients suffering from coronary heart disease with blood stasis syndrome and those who have coronary heart disease without blood stasis syndrome and to develop the relationship between blood stasis syndrome and EPC subsets,CEC subsets and EMP.Method:96 patients with coronary heart disease and angina pectoris was selected from Department of Cardiology,Guanganmen Hospital of China Academy of Chinese Medical Sciences from November 2016 to January 2017,a total of 18 healthy volunteers were selected as the control group from Medical Center of of Southern District of Guang'anmen hospital.Referring to the 2012 "study on the diagnostic critetia for coronary heart disease patients of blood stasis syndrome",the patients with coronary heart disease were divided into blood stasis syndrome group and non blood stasis syndrome group.According to the American ACC/AHA "guidelines for the diagnosis and treatment of chronic stable angina" and"guidelines for the diagnosis and treatment of unstable angina and non ST elevation myocardial infarction",the angina patients were divided into stable angina group and unstable angina pectoris.In the early morning fasting state,venous blood was collected from cubital vein,and the blood was saved by EDTA anticoagulation.Flow cytometry was used to measure the levels of various subsets of endothelial progenitor cells and circulating endothelial cells.Platelet free plasma(PFP)was prepared by gradient centrifugation.Endothelial microparticles in PFP was also detected by flow cytometry.Then we compared the differences of the above indexes in patients with coronary heart disease and the healthy people,and the difference in patients with different types of angina pectoris,as well as patients with blood stasis syndrome and those without blood stasis syndrome,and analyzed the correlation between EPC subsets,CEC subsets,EMP and blood stasis syndrome of coronary heart disease.Results:1.The level of CD34+CD309+EPC,CD34+CD309+CD45dimEPC and CD34+ CD309+CD45dimCD14+EPC in patients with angina pectoris were higher than those in healthy group,the level of CD34+EPC decreased,the difference was statistically significant(P<0.05);There was no significant difference between patients with angina pectoris and healthy group in the level of CD309+EPC(P>0.05).The level of CD146+CEC,CD146+CD45 CEC,CD146+CD45-CD106+CEC in patients with angina pectoris were higher than those in healthy group,the difference was statistically significant(P<0.05).The level of CD31+CD42b-EMP was higher in patients with angina pectoris than those in healthy group,the difference was statistically significant(P<0.05).2.The level of CD34+CD309+CD45dimEPC and CD34+CD309+CD45dimCD14+ EPC in patients with unstable angina pectoris were higher than those with stable angina pectoris(P<0.05).The levels of CD34+EPC,CD309+EPC and CD34+CD309+EPC were not significantly different in patients with unstable angina pectoris when compared with patients with stable angina pectoris(P>0.05),the level of CD146+CEC and CD146+CD45'CEC were significantly higher in patients with unstable angina pectoris than those with stable angina pectoris(P<0.05).There was no significant difference in the level of CD146+CD45-CD106+CEC between patients with unstable angina pectoris and stable angina(P>0.05).The level of CD31+CD42b-EMP was higher in patients with unstable angina pectoris than those with stable angina pectoris,the difference was statistically significant(P>0.05).3.The level of CD34+CD309+EPC,CD34+CD309+CD45dimEPC and CD34+ CD309+CD45dim CD14+EPC in patients with coronary heart disease and blood stasis syndrome were higher than those without blood stasis syndrome(P<0.05).There was no significant difference in the level of CD34+EPC and CD309+EPC between patients with blood stasis syndrome and non blood stasis syndrome(P>0.05).The level of CD146+CEC,CD146+CD45 CEC,CD146+CD45-CD106+CEC were higher in patients with coronary heart disease and blood stasis syndrome than those without blood stasis syndrome,the difference was statistically significant(P<0.05).The level of CD31+CD42b'EMP was higher than that of non blood stasis syndrome of coronary heart disease(CHD),the difference was statistically significant(P<0.05).4.The level of CD31+CD42b-EMP of patients with coronary heart disease was positively correlated with CD34+CD309+CD45dimEPC and CD146+CEC.Blood stasis syndrome and the level of CD31+CD42b-EMP,CD34+CD309+EPC,CD34+CD309+CD45dimEPC,CD34+CD309+CD45dimCD14+EPC,CD146+CD45-CEC,CD146+CD45-CD106+CEC have positive correlation(P<0.05).5.Logistics regression showed that CD31+CD42b-EMP and CD34+CD309+EPC were closely related to blood stasis syndrome.Conclusion:1.The level of CD34+EPC decreased,the level of CD34+CD309+EPC,CD34+CD309+CD45dimEPC,CD34+CD309+CD45dimCD 14+EPC,CD 146+CEC,CD 146+CD45-CEC,CD 146+ CD45'CD 106+CEC,CD31 +CD42b-EMP rose in peripheral blood of coronary heart disease patients.2.The level of CD34+CD309+CD45dimEPC,CD34+CD309+CD45dimCD14+EPC,CD146+CEC,CD146+CD45-CEC and CD31+CD42b-EMP rose in patients with unstable angina pectoris.3.The level of CD34+CD309+EPC,CD34+CD309+CD45dimEPC,CD34+CD309+CD45dimCD 14+EPC,CD 146+CEC,CD 146+CD45-CEC,CD 146+CD45 CD106+CEC and CD31+CD42b-EMP went up in patients with coronary heart disease with blood stasis syndrome.4.The level of EMP was positively correlated with the level of CD34+ CD309+ CD45dim EPC and CD146+CEC in patients with coronary heart disease.5.The blood stasis syndrome of coronary heart disease was positively correlated with CD34+EPC,CD34+CD309+EPC,CD34+CD309+CD45dimEPC,CD34+CD309+CD45dim CD14+EPC,CD146+CD45-CEC,CD146+CD45-CD106+CEC,CD31+CD42b-EMP and CD42b+PMP levels in patients with coronary heart disease(CHD).Among them,the level of CD31+CD42b-EMP and CD34+CD309+EPC had the most closely relationship with blood stasis syndrome,which may be a predictor of coronary heart disease with blood stasis syndrome.
