Clinical Experience Of Treating Thyroid Nodules With Prof. Shi Yan, A Study Of Pathogenesis And Treatment Of Thyroid Nodules

Purpose:To explore my mentor Professor Shiyan’s clinical theoretical thoughts on syndrome differentiation and treatment of thyroid nodule based on pathogenesis and to summarize, conclude and demonstrate his clinical experience.Material and method:Through learning from Professor Shiyan clinical practice and accumulating related typical cases; Under his guidance, to compact his theoretical knowledge on etiology and pathogenesis of thyroid nodule and conclude thinking mode of syndrome differentiation and treatment of this kind of disease and law of making prescription and combination usage of herbs, by summarizing cases showed efficiency.Results:1.It ought to inherit theoretical thoughts of TCM and keep up to modern medical theories to study pathogenesis of goiter(thyroid nodule) and follow etiology, pathogenesis, syndrome and treatment respectively, nerve confusing medical knowledge. Emotional diseases are the main cause of goiter.2.The initial pathogenesis of goiter is qi depression, then stagnation of phlegm and blood stasis formed, each of them would tend to be hyperactive or deficient more or less. The only way to determine therapeutic method is to recognize the root cause of disease.3.There are four ways to cure goiter: the first one is to dissipate depression; the second one is to eliminate stagnation; the third one is to activate collaterals and the fourth one is to regulate qi and blood via acupuncture. The only way to learn the law of treating disease is to know the order, which can be obtained by syndrome differentiation and treatment.Conclusion:1. The exploration of etiology and pathogenesis of thyroid nodule in TCM should focus on internal injury caused by excess of seven emotions, and pay attention to improper diet, environmental inadaptability and physical factors.2. The basic pathogenesis of thyroid nodule is qi stagnation, phlegm retention and blood stasis, among which need to distinguish the primary and secondary.3. The therapeutic method of thyroid nodule mainly follow the four principles that dissipating depression, eliminating stagnation, activating collaterals and regulating qi and blood by acupuncture, meanwhile varying the composition with symptoms.
