Medical care insurance is an important component of social guarantee system, it is also the complicated item among various insurance systems, so it is called"international problem". The reform of medical care insurance is a complex social systematic project. Which is closely related the general situation of reform, development and stability. People are paying much more attention to this problem than to others, because it involves the adjustment of multilateral benefits. At present, the basic medical care insurance system which combines society's balance and personal account in cities and towns in China has just begun. However, it still has a hard and long way to go compared with endowment and unemployment insurance.Beginning from the revolution and background of present social medical care insurance system of both domestic and abroad, the thesis analyses the present situation of medical care insurance reform, and generalizes a series of successful experience and failure lessons of medical care insurance system from abroad. According to our national situation, the thesis even propose a policy to perfect the medical care insurance system, on center of keeping medical expenses down, at the guidance of protecting the practical interest of cities and towns workers or eliminating the main obstacle in social medical care insurance reform.The thesis is composed of four parts in all. Firstly, reviews and analyses the evolution and present situation of the medical care insurance of cities and towns workers; Secondly, analyses the objectivity which restricts the medical care insurance reform, and then finds a solution to breakthrough the bottleneck of social medical care insurance reform; Thirdly, analyses the evolution of medical care insurance reform of Western countries objectively, so that we can make a strong argumentation why we should learn their successful experience to perfect our medical care insurance; last, makes a serious investigation towards some major problems which needs to be solved during our present medical care insurance reform, such as perfect the system, enlarge the coverage of insurance in a high speed, promotes the medical and medicine administration system at the same time sets up a human—oriented concept called"equality priority and efficiency"The thesis sticks to the basic principle of dialectical and historical materialism in investigation method, tries to adopt a series of methods, includes the followings:Scientific abstraction, statistics, the combination of international comparison and internal analysis, etc.The thesis has already made some achievements after making a thorough study about the obstacle and policy of our national medical care insurance reform.
