Objective Observing the effect of gynostemma pentaphylla powder on renal cell apoptosis and related gene Bcl-2,Bax in diabetic nephropathy rats, to find out the possible mechanism of the therapeutical effect of gynostemma pentaphylla powder to diabetic nephropathy.Methods Using unilateral nephrectomy plus STZ injection improved rat model for diabetic nephropathy copy, the experimental rats were divided into normal control group, model control group, gynostemma granules of high dose group, gynostemma granules of middle dose group, gynostemma granules of low dose group and valsartan group, observe the general state of health, use metabolic cages in order to retain an 24-hour urine at 2 weeks and 4 weeks of treatment and detecte the urinary creatinine and urinary albumin. The rats were killed at 28 days after giving medicine, then blood specimen was collected from the main abdominal vein to examine blood glucose, blood fat, serum creatinine and blood urea nitrogen. Pathological changes of renal were observed, Apoptotic cells in renal cortex were detected by means of terminal-deoxynucleotidyl transferase mediated d-UTP nick end labeling (TUNEL). The expression of apoptosis-related genes Bcl-2 and Bax in rat kidney tissue was measured by pathomorphology and immunohistochemistry.Result 1. Blood glucose:the values of blood glucose of model group and the treatment groups were significantly higher than normal group(P<0.01), after 28 days'treatment, the values of blood glucose of each dose of gynostemma granules group were decreased, and compared with valsartan group and model group, there were significant differences(P<0.01); 2. Blood fat:the values of triglyceride, total cholesterol and low-density lipoprotein of each dose of gynostemma granules group were all significantly lower than model group and valsartan group(P<0.05 or P<0.01). especially for gynostemma granules of high and middle-dose group on lowering triglycerides and total cholesterol; 3. Renal function:the treatment groups'serum creatinine, blood urea nitrogen and endogenous creatinine clearance rate were obviously dreased. Compared with model group there were significantly differences (P<0.05 or P<0.01); The urinary volume and 24h urinary albumin of treatment groups were significantly lower than the model group(P<0.01), especially for gynostemma granules of high dose group; The renal hypertrophy index of treatment groups all lower than model group(P<0.05 or P<0.01), there was no significant difference in each treatment group; 4. Apoptosis-related genes:compared with model group, the treatment groups had less positive detection rate of renal cell apoptosis(P<0.01), higher expression of Bcl-2 and lower expression of Bax(P<0.05 or P<0.01), especially for gynostemma granules of high dose group.Conclusion Gynostemma granules can regulate glucose and lipid metabolism in rats with diabetic nephropathy, reduce the urinary albumin, improve the renal function and can be used for the treatment of early diabetic nephropathy, The mechanism might increase Bcl-2 expression, reduced Bax expression to interfere with diabetic nephropathy rat kidney cell apoptosis.
