Objective:This research uses PCR-RFLP to study the relationship between GDM and the polymorphism of TNF-αpromotor together with the TNF-αlevel of blood plasma. At the same time, the study explores the relationship TNF-αallele with body mass index (BMI), plasma levels of TNF-αand insulin resistance (IR).Method:Seventy-eight women were enrolled into the study group (included 19 women who were GIGT), seventy-eight healthy women were chosen at random as the control group. Exclusion criteria was the presence of infection, inflammatory, hypoghyroidism, endocrine disease other than diabetes, or the use of anti-obesity drugs. Four milliliters venous blood was taken from all women in the two groups. Genomic DNA was extracted from venous blood. By using PCR-RFLP, we detected the distribution of TNF-a promotor alleles frequency in these subjects. Plasma TNF-αlevels and insulin levels were measured by ELISA.Results:1. There was a significant increase of "A" alleles frequency of TNF-αpromotor-308 gene in GDM women compared with the controls (60.3% vs 29.5%, P<0.05) and an increase of the "AA+AG" genotype were observed in GDM group (73.5 vs 38.3%, p<0.05).2. The women who suffered from GDM had a higher BMI (24.8±0.8 vs 23.5±0.7, P< 0.01), plasma TNF-αlevels (5.4±1.4 vs 3.6±1.3, P<0.01) and a lower insulin sensitivity: HOMA-IR (5.8±1.4 vs 3.5±1.0, P<0.01) than those controls.3. In GDM group, the patients carrying the "GA+AA" genotype had a higher BMI (23.7±0.4 vs 21.6±0.7, P<0.05), plasma TNF-αlevels (5.65±0.61 vs 3.02±1.14, P<0.01) and a lower insulin sensitivity:HOMA-IR (5.6±1.6 vs 3.8±1.2, P<0.01) compared with those carrying the other genotype "GG"4. The relationship of each two of the biochemical indicators:BMI, TNF-αand HOMA-IR is directly correlate.Conclusion:1. There were-308Gâ†'A polymorphism of TNF-αpromotor gene in the Han nationality women in Shandong Province.2. The alleles "A" of TNF-αmay be regarded as having the susceptibility of GDM women in Shandong Province.3. TNF-αpormotor-308"GA+AA" genotype may relate with the TNF-αlevel of blood plasma and may strenghten insulin resistance.
