Objective: To explore the relationship between heart rate recovery (HRR) and Coronary artery Disease and clarify the related factors of HRR after treadmill exercise test.Methods: 178 patients with suspected coronary artery disease who received symptom-limited treadmill exercise test and coronary angiography were enrolled and divided into normal HRR group and abnormal HRR group based on the status of HRR at one or two minutes after treadmill exercise test ((N=109 VS. N=69)). The baseline characteristics, parameters of treadmill exercise test and coronary angiography results of the two groups were compared. Further more, related factors of HRR after treadmill exercise test were evaluated by analysis of Logistic regression model based on above explored results.Results: (1)There were much more females, smokers, and patients with diabetes mellitus and ST-T changes at rest in the group of abnormal HRR, compared with the group of normal HRR (all P values<0.01). (2) The patients had higher heart rate at rest and more cases exhibited ST segments deviation, exercise-limited angina, lower level of peak heart rate attained, shorter exercise duration and lower Duke test scores in abnormal HRR group compared with the contrast group(P values<0.05 or P values<0.01). (3)The cases with negative results of coronary angiography in the abnormal and normal HRR group were 24(34.78%) and 73(66.97%) respectively(P values<0.01). The patients with significant coronary artery lesions were 29(42.0%) and 26(23.9%) in the abnormal and normal HRR group respectively, severe coronary artery lesions were 16( 23.2%) and10(9.2 %) respectively(P values<0.05 or P values<0.01); The Gensini scores were significantly higher in group of abnormal HRR than the group of normal HRR (P values<0.01). Spearman coefficient of correlation between abnormal HRR and severity of coronary artery lesions was 0.32(P values<0.01); But the correspondence analysis did not indicate there was a corresponding relationship between abnormal HRR and localization of coronary artery lesions(.4)Statistics based on Logistic regression model indicated that sex, impaired glucose tolerance or diabetes mellitus, resting heart rate and coronary artery lesions had influence to HRR after treadmill exercise test.Conclusion: The abnormal heart rate recovery after treadmill exercise test is associated with the severity, but not the localization of coronary artery lesions. Sex, impaired glucose tolerance or diabetes mellitus, resting heart rate and coronary artery lesions maybe have influence to heart rate recovery after treadmill exercise test.
