Correlation Between Expression Of Maspin And Bax With Efficacy Of Neoadjuvant Chemotherapy For Cervical Carcinoma

[Objective]Through observing and detecting the expression and change of maspin and bax before and after neoadjuvant hemotherapy in cervical carcinoma, the purpose of the study is to investigate the expression and change of maspin and bax in cervical carcinoma with neoadjuvant sensitivity, curative effect,clinical pathology and so forth. To make neoadjuvant chemotherapy individualization, it can prevent from delaying operation and radiotherapy because of useless and unfine chemotherapy. Looking for a new index to forecast sensitivity and to appraise curative effect of neoadjuvant chemotherapy in cervical carcinoma.[Methods]There are 30 patients with cervical cancer FIGO stage I a toⅢb treated with NACT. The expression of maspin and bax is checked by immunochemistry LSAB method, and analyze the relationship between the expression of maspin protein,bax and the NACT effect as well as the relationship between the expression of maspin protein,bax protein and clinic pathological factors such as FIGO stage and pathological grade[Results](1) there is a significant difference between cervical caner group with control group for higher expression of maspin and bax.(2) After NACT,30 patients have gain curative effect, the total clinical response ratio of NACT is 83.3%(25/30),36%of patients(9/25)with complete remssion and patients 64%(16/25)with partial remission is recorded, and compared with (16.7% 10/64) of patients with stable disease.(3) The expression of maspin and bax before and after NACT:Comparing with the expression of positive cell of maspin and bax show increase in before and after NACT significantly in partial remission cases(p<0.01).In complete remission, stable disease cases and progressive disease the expression of maspin and bax after NACT has no obvious change(P>0.05)(4) Compared with different FIGO stages(Ⅰa～Ⅲb) and pathological grade:there is significance expression of maspin and bax before and after NACT inⅡb-Ⅲb group(P <0.05).no signifeicance is found expression of maspin and bax before and after NACT in I b-Ⅱa group. no signifeicance is found expression of maspin before and after NACT in different pathological grade group(p>0.05). there is significance expression of bax before and after NACT in high and middle grade group(P<0.01). and no signifeicance is found expression of bax before and after NACT in low pathological grade group(p>0.05).(5)the relationship of expression of maspin and bax before and after NACT:negative and positive expression of maspin and bax before and after NACT is 90% (18/20) and 30% (3/10),there is significance compared with positive expression of maspin and bax before and after NACT.(6) there is significant direct correlation between the expression of maspin and bax in these patients before and after NACT(before r=0.196, p>0.05;after r=0.617, p<0.01)[Conclusion]1.The arterial perfusion neoadjuvant chemotherapy is an effective means to cure the cancer of the cervix,and with the way of neoadjuvant hemotherapy, which includes Cisplatin have good curative effects for 1～2 course of treatment. 2 The expression of maspin and bax protein is not assiociated with stage and grade of pathology,therefor,as a independent factor,maspin and bax protein can predict the curative effects of NACT, The more highly maspin and.bax express before NACT, the better NACT can gain curative effect.3.maspin and bax has sensitiveness to reflect curative effect and conduct therapy...
