Background: Cervical cancer is a serious disease which hazards to the health of patients. According to the epidemiological data and combined with the global data show that cervical cancer is the common malignant tumor of gynecological cancers which rank only second to the breast cancers in global. The mortality of cervical cancer is the third status. Cervical cancer is the commonest malignant tumor of gynecological cancers in our country. The patients are making more youthful. Many patients failing to timely discover disease and delay treat it. While neoadjuvant chemotherapy provides the surgery opportunity for many patients with cervical cancer. Significantly increased tumor resection rate. Neoadjuvant chemotherapy with paclitaxel plus platinum-based drugs for cancer patients preoperative and postoperative applications. The early view displays that the tumor cells draw support the already formed lymphatic to transfer but with the deepening of the study, tumors can induce lymphangiogenesis thus promoting metastasis of tumor cells. So the tumor lymphangiogenesis in cancer metastasis has become a new hotspot. Due to the lack of specific lymphatic markers, the study of lymphatic vessels in the tumor lymphangiogenesis are less. Some researchers labeled lymphatic with anti Podoplanin obtained success. The lymphatic markers podoplanin were first discovered in 1999. Podoplanin is considered an ideal marker for lymphatic. So the tumor lymphangiogenesis in cancer metastasis has become a new hotspot. The podopianin emergency done a very good bedding to study microlymphatic deeply.Maspin’s appear was set up the comparing of the normal breast tissue and breast cancer. It has many fuctions, as inhibiting tumor growth,inhibiting tumor invasion, inhibiting tumor metastasis and inhibiting tumor angiogenesis, et al.Maspin expression is complex, it is highly expressed in breast, prostate and other normal tissue cells. Maspin is not expressed in the thyroid tissue. Maspin is low or no expression in normal ovarian tissue, It showed high expression of maspin state in ovarian cancer tissues. Many studies have shown, maspin is a fully functional tumor suppressor gene. It is highly expressed in normal cervical tissue,but in cervical tissue maspin expression was significantly reduced. Microvessel and lymphatic microvessel are the same occurrence of homology. Both take place differentiation in a different regulation mechanism. So it can make a question that whether is a correlation between changes in cervical squamous cell carcinoma maspin expression and tumor lymphatic microvessel density after Neoadjuvant chemotherapy.Objective: To discuss the expression of antioncogene maspin and tumor lymphangiogenesis before and after neoadjuvant chemotherapy(NACT) for squamous carcinoma of the cervix. To explore the relationship between maspin and tumor lymphatic system in inhibiting cancer progression.Methods: Selected 55 cases of Ib2-IIa stage cervical squamous carcinoma from Department of Pathology in BinZhou Medical University Hospital , and collected specimen before and after chemotherapy(TP chemotherapy), then detected the expression of maspin and tumor lymphangiogenesis of cervical cancer by immunohistochemistry. To analyze the relationship between maspin and tumor lymphangiogenesis after neoadjuvant chemotherapy. To analyze the clinical efficacy of curing the cervical squamous carcinoma by neoadjuvant chemotherapy and the relation with age and FIGO stage.Results: Immunohistochemistry showed that the expression of maspin was significantly increased(P<0.05) after neoadjuvant chemotherapy, while the LMVD after chemotherapy(4.56±2.21 a/mm2) was more significantly decreased than before(8.19±4.17a/mm2 )(P<0.05). Increased maspin expression showed significantly negative association with decreased LMVD after neoadjuvant chemotherapy. The clinical total effective rate of neoadjuvant chemotherapy in the treatment of cervical squamous cell carcinoma was 81.82%, and the neoadjuvant chemotherapy showed no statistical differences in age and FIGO stage(P>0.05).Conclusion: Neoadjuvant chemotherapy may be not associated with age and FIGO stage. The clinical total effective rate of neoadjuvant chemotherapy in the treatment of cervical squamous cell carcinoma was 81.82%. The expression of maspin is significantly increased and the tumor lymphangiogenesis decreased after chemotherapy. Increased maspin expression showed significantly negative association with decreased LMVD after neoadjuvant chemotherapy. Up-regulation of maspin’s expression may do help to control the formation of the tumor lymphatic system. It’s possible to control the development,invasion and transformation of cancer cells. It may be one of the mechanisms which neoadjuvant chemotherapy inhibites the cervical cancer progression. |
