BackgroundIn the present society,with the arrival of expediting of People's work-rhythm, enlarging communication,fierce competition and increasing pressure,stress in vocational is more evident.Unlike the way that the biological,physical and chemical factors cause specific occupational disease,occupational stress may damage our health by a non-specific way.Stress can result in fatigue,anxiety,repression,changes of immune function and even cause diseases.According to modern strain theory, hypothalamic pituitary adrenal axis(HPA) plays an important role in the process of impaired immune system caused by stress,degree of activation is affected by many factors and has a significantly individual difference.Individual's replication ability plays a decisive role.Confirmed by present experiments,macrophages and monocytes can be activated and release pro-inflammatory cytokines,including IL-1,IL-6,TNE These pro-inflammatory cytokines can not only regulating immune but also affected immune function and induced hypothalamic pituitary adrenal axis.With the arrival of further development in cell and molecular biology,it is an inevitable trend that Occupational Stress research in molecular level instead of behavior level.The difference of genetic background involves many genetic factors,results in multiple gene comprehensive effect.It hasn't any reports both at Home and Abroad,so the first step to research, we have to from the three most close cytokines relatived with neuro-immune system.ObjectivesOn the basis of analysis of literatures,we selected TNF-A -308 and -238 sites to investigate the effection of TNF-a changes by G/A in single nucleotide polymorphism of -308 and -238 sites in occupational stress.And explore the relation between occupational stress and immune function measured by TNF-a.MethodsAn electrical appliance factory was selected by cluster sampling.All of the 178 staff working in the pipeline in the factory were surveyed,166 questionnaires received, including 126 males and 43 females,the validation of questionnaire is 93.3%. Occupational Stress test scale(including stress factors scales,stress reaction scales, personality scales,stress remission factors scales) was used to investigate occupational Stress factors,stress reaction,personality and stress moderating factors, the degree of stress was measured by ERI(ERI=pay/(return×0.5454)).Staff with ERI scores that higher than the ERI score of the point of lower 25%of the distribution of the ERI scores were treated as low strain groups,while the other treated as high strain groups.Nucleotide polymorphism of -308 and -238 sites were tested by the technology of PCR-RFLP.Concentration of TNF-a in serum was detected by the technology of radioimmunoassay.The software of SPSS 12.0 was used to analyze the data.Results1 Internal control and drive have impact on concentration of TNF-a in serum. Concentration of TNF-αin serum is negative correlation with the scores of role ambiguity(r=-0.167,P=0.035),decided power(r=-0.168,P=0.034),sex (r=-0.183,P=0.021),professional title(r=-0.225,P=0.009) and vocational (r=-0.160,P=0.044).Results of multiple stepwise regression analysis shows that coping strategy,role ambiguity,psychological needs/decide freedom and ERI are predictor of concentration of TNF-αin serum. 2 There was no statistical significance between the concentration of TNF-αin serum of workers with A allele and the concentration of TNF-αin serum of workers with G allele(P>0.05).3 There was no statistical significance of the difference between the distribution frequency of A allele in TNF-α(-308) of low stress group and the distribution frequency of A allele in TNF-α(-308) of high stress group(χ~2=0.0001,P=1.0). The distribution frequency of A allele in TNF-α(-238) of low stress group and the distribution frequency of A allele in TNF-α(-238) of high stress group were different statistically(χ~2=5.449,P=0.020).Conclusions1.Occupational stress has impact on the concentration of TNF-a in serum.2.There was no difference between the concentration of TNF-αin serum of TNF-α(-308) and(-238) sites mutation group and the concentration of TNF-αin serum of TNF-α(-308) and(-238) sites non-mutation group.3.TNF-α(-238) site was a site with single nucleotide polymorphism,A/A genotype may be the non-susceptible genotype of occupational stress,G/G genotype may be not the susceptible genotype of occupational stress;TNF-α(-308) site was also a site with single nucleotide polymorphism,but it may be uncorrelated with genetic susceptibility in occupational stress.
