Diabetic patients with diabetic nephropathy is the most commo vascular complication of end stage renal failure also contributed to an important reason. Performance of the basic pathological changes of glomerular extracellular matrix accumulation, mesangial widening, basement membrane thickening, until the development of the glomerulosclerosis and renal interstitial fibrosis.The characterize is a large number of clinical appearance of proteinuria and is closely related the severity of diabetes. The emergence of large quantities of proteinuria not only to further increase the glomerular filtration membrane damage, leading to glomerular sclerosis, but also to promote proteinuria in renal tubular epithelial cell uptake of certain harmful ingredients, resulting in tubular damage and Renal interstitial fibrosis, and eventually lead to kidney failure.Glomerular visceral epithelial cells which also called podocytes play an important role in the process of the occurrence of proteinuria. Studies show that people and experimental diabetic nephropathy are related to apoptosis and shedding of the podocytes, and the podocytes once damaged will be off the lack of ability to regenerate after. After the cells were damaged enough to reduce the number and density, while the remaining podocytes to cover the bare GBM (Glomerular basementmembrane, GBM) arisingfrom compensatory hypertrophy, foot process broadening, fusion, so that Increased permeability of the glomerular filtration barrier, leading to the occurrence of massive proteinuria. Nephrin as the main component of the hole diaphragm involved in cell signal transduction, and to maintain normal podocyte morphology and function, its gene and (or) abnormal protein expression in the pathogenesis of proteinuria has a very important role. In conclusion, podocyte injury led to continuous progress in the DN, nephrin in the hole diaphragm of podocytes plays an important role on the selective function of the glomerular filtration barrier, the further research in this field may lead to the pathogenesis and diagnosis of DN Therapeutic breakthroughs.According to the hypothesis of the proposed supervisor Professor Li Jianmin " Kidney meridians unreasonable" theory which is the progression of chronic kidney disease, choose the instructors of clinical experience treating chronic renal failure side CTLBSP (Compound Tongluo Baoshen Prescription,) For a series of experimental diabetic nephropathy studies. The CTLBSP is composed of 8 kinds of Chinese herbal:Heterophylly Falsesatarwort Root, Salviamiltiorrhiza,Glossy Privet Fruit, Leech, Rhubarb, Twotoothed Achyranthes Root and Notoginseng etc.It can invigorate the kidney and drive away the evil, promote the blood circulation to remove meridian obstruction. This article started this series of studies designed to explore the side of mechanism on diabetic nephropathy.1 ObjectiveIn order to offer accurate and scientific experimental evidences to the clinic prevention and cure of DN, this experiment carries through the study by taking drug serum as the medicine added LPS as a stimulus by blood serum pathological method which was improved, then investigates further the effect of Compound Tongluo Baoshen Prescription on expression of nephrin secreted by cultured rats Glomerular visceral epithelial cells.2 Methods2.1 To select podocytes (Glomerular visceral epithelial cells)after it is stimulated by LPS, the rats were given intervention of CTLBSP with drug serum. Check the proliferation of the podocytes by MTT methods at 12h, 24h, 36h and 48h. 2.2 To select podocytes (Glomerular visceral epithelial cells) and after it is stimulated by LPS, the rats were given intervention of CTLBSP with drug serum. The nephrin protein was detected in rats renal tissue by immune histochemistry staining and western blot.3 Results3.1 LPS can evidently promote the proliferation of podocytes (Glomerular visceralepithelial cells). CTLBSP drug serum can restrain the proliferation of the podocytes stimulated by LPS. 3.2 CTLBSP can prominently increase the expression of nephrin.4 Conclusions4.1 CTLBSP can inhibit the proliferation of podocytes4.2 CTLBSP can prominently increase the expression of nephrin and reduce proteinria.
