Hypertension Syndrome Differentiation Type Correlation With Risk Factors And Target Organ Damage

Purpose:Research the relation between TCM syndrome and risk factors and damage of target organ.Material and method:The study objects of this papers were chosen from 100 hypertension inpatients of cardiology from March 2009 to May 2010. Using EpiData3.1produced retrospective questionnaires symptom-complex of hypertension. After collecting of medical records, the information was inputted to questionnaires. Analyzing the data by SPSS17.0 is to research the relation between TCM syndrome and risk factors and damage of target organ.Results:1. TCM symptom-sign and syndrome: Yin deficient positive high (34) > high abundant of liver-fire (25) > phlegm-dampness (19) > deficiency of yin and yang (15) > blood stasis (7). In common symptoms and syndrome distribution of hypertension, dizziness, headache, fatigue, nausea, soreness and weakness of lower back, tinnitus, irritable, constriction in chest, dry mouth and thirsty, bitter taste in mouth, weakness of knee have significant differences of statistics (P<0.01), anorexia has differences of statistics (P<0.05).2. Analyzing risk factors of hypertension: In different syndrome the ration between male and female has no difference of statistics, but the ration in different age stage has significance of statistics (P<0.05). In obesity, smoking, drinking, family history of hypertension, the ratio in different syndrome has significance of statistics (P<0.05). Metabolism of blood sugar has significance of statistics in different syndrome (P<0.05). Total cholesterol (CHO) and low-density lipoprotein (LDL-C) has significance of statistics (P<0.05).3. The level, rating and stratify of risk in hypertension: The level of SBP in different syndrome has significance of statistics (P=0.001), but the level of DBP has no significance of statistics (P=0.279). Both rating and stratify of risk have significance of statistics (P<0.05).4. Analyzing damage of target organ in hypertension: Merging the damage of heart, brain and vascular have difference of statistics (P<0.05), but merging the damage of kidney has no difference of statistics (P>0.05). LA, LVDd, LVSd,LVPWd and Serum creatinine (Cr) in different syndrome have difference of statistics (P<0.05).Conclusion:1. The distribution of TCM syndrome has regularity: The distribution of Yin deficient positive high is the most extensive, the else are high abundant of liver-fire, phlegm-dampness, deficiency of yin and yang and blood stasis in turn.2. Common symptoms in four examination techniques of TCM: dizziness, headache, insomnia and fatigue are the most common. In tongue characteristic distribution pattern, the reddened or pale tongue, yellow or thin fur, wiry and thready pulse or wiry and rapid pulse are the most common.3. The rating and stratify of risk of hypertension in different syndrome have significant difference: The rating ofⅠ,Ⅱand medium risk, higher risk distribute in blood stasis and high abundant of liver-fire in common. The rating ofⅢand the highest risk distribute in Yin deficient positive high and phlegm-dampness in common.4. The risk factors, objective indicators in hypertension and TCM syndrome have clear relationship: Before the age of 60 high abundant of liver-fire and phlegm-dampness are the most. After the age of 60 deficiency of yin and yang and blood stasis are the most. Obesity and drinking distributing in phlegm-dampness is the most, smoking distributing in high abundant of liver-fire is the most, family history of hypertension distributing in blood stasis is the most. Merging abnormal blood sugar distributing in Yin deficient positive high is the most. Merging abnormal cholesterol distributing in phlegm-dampness is the most.5. The damage of target organ and TCM syndrome have clear relationship: Merging the damage of heart and brain distributing in blood stasis is the most. Merging the damage of kidney distributing in Yin deficient positive high is the most. Merging the damage of vascular distributing in phlegm-dampness is the most.
