| Cholelithiasis is a worldwide disease with a high incidence and does harm to human physical and mental health.With the changing of population and dietary habit throughout the world, cholelithiasis is becoming more common.There are three clinical types of gallstones including cholesterol stones pigment stones and combination calculus.In our nation CS accounts for the higher percentage, while the incidence of BS is only 10 percent. In recent 20 years, mordem research of the therapy has been increasing gradually, but it remains some defects: (1)Pigment stones, to a large extent is hepatolith, always coherent with bile duct stenosis.It raises the operative difficulties. (2)The rate of postoperative remaining stone is high and it's easy to reoccur and to be infected. (3)DMSO and some dissosations are commonly used to pigment stones by perfusion.Research on dissolving gall stone is being carried out, but no ideal drug has been found out until now.Therefore, nonoprative therapies still have potentiality to study, especially in TCM.Da-chaihutang-jiajian-chongji (DCHT), a Chinese TCM preparation, which has been clinicly proved have the capacity to exclude stone and to prevent and cure Cholesterol stones.In order to asertain its further eflfection and mechanism to pigment stones, we have studied it through series experiments,which was carried out in four groups: normal group, model group prevention groups and curative groups.This paper falls into two parts: literature study and experiment research.The literature study deals with the epidemiology of cholelithiasis.And a historical retrospect is given according to the knowledge of gallbladder in pathology and physiology,which follows a discussion about the formation mecanism of pigment stones.Besides, a comprehensive nonoperative TCM therapies is described,and the relation between hyperinsulinsium and the gallbladder stone is discussed.In the first part of the experiment, we succeeded in forming the pigment stones in pig-ginues in accordance with foreign literature.Then, we feed animals DCHT in three layer doses to observe the effection,and inspect the concentration of Ca ion TBILand DBIL by means of the biochemical methodln the treatment part of the experiment, we set CDCA as contrastion.Then, through HE dying, we observe the pathological changing of hepatic epithelial cell layer carefully.At last, acute toxic expriment has been conducted for the clinical safety.Through the morphogenetic research, the formation of pigment stones has been set up from the continous intradermal injection with dihydrochloride.The rate of Dihydrochloride induced pigment stones is up to 90%.The result indicated that the prevention groups have the obvious effection of dissolving and excluding stones and also can restrain the formation of bilestone effectively, which markedly in large dosage group compared with Chenodeoxycholic acid.The effect of curation is moderate while its prevention effect is remarkable..From the morphologic observation, we successfully establish the model.The gallbladders of the model group look rough with the dermal cell infiltrated by acute inflammatory cell.The mucosadifferentiated into multilayers.We can see from prevention groups that the dermal cell well-arranged and the mucosa differentiated slightly with little secration.No obvious inflammation was found between stroma and muscle layer.In the curative groups, the mucosa differentiated into moderate multilayers,and the dermal cell coarse-arranged and the mucosa differentiated with some secratioalnfalmmatory infltration can be seen between stroma and muscle layer, and inflammatory cell accumulated in submucosaBut it is clear that to some extent,some repair function of curative group ourweight that of the model group.On the base of the experiment,the best clinical opportunity of gallstones could be saughtThe result of the experiment indicates that the high risk group with gallstone may include the people with hyperglucose hyperchlosteral and hyperlipidemia.ATE implied that the maximum tolerance dose of DCHT... |
