Objectives: Near 60% epilepsy seizures in sleep. There isn,t the conclusionof this kind of phenomenon still now. Know to all,there is the notable defferencelevel of adrenocorticontrophin and cortisol in day and night. Is there the relationof these two kinds of endogenesis hormone with the sleep epileptic seizures ?Probe into the relation of the level of ACTH and COR and the instantaneousEEG features in the early stage of sleep epileptic seizures. Probe into therelation of the level of ACTH and COR around sleep epileptic seizures. Toexplore the instantaneous EEG features in the early stage of sleep epilepticseizures Methods:Select patients of epilepsy and hysteria in epilepsy out-patient ofthe first hospital.Before the experiment volunteers and their family know themeaning of experiment and agree into set.There is the contract of the patientsinducted by the medicine.There are two groups,one is the epilepsy group(28patients are nature goes into seizure,8 patients are inducted with medicine);oneis the control group (11 patients are hysteria). The EEG changes and seizuremanifestations of patients with sleep epilepsy were monitored by Video-EEGmonitoring system.Serum ACTH and COR were determined by LIA. Adopt therepeated measure square statistical analysis, P<0.05 . Results:1,The results of ACTH and COR: (1)Compare in group:① Thereis notable difference with the level of serum ACTH and COR around seizures inepileptic group: ACTH( F=20.925, P<0.001), COR.( F=42.324, P<0.001).②There is notable difference with the level of serum ACTH in defferent focusof infection groups: ACTH ( F=4.577, P=0.018). The changed level of serumACTH is notable in temporal lobe.③ There isn,t notable difference with thelevel of serum ACTH and COR around seizures in inducted with medicinegroup and case group: ACTH( F =0.001, P= 0.978),COR(F=0.397, P= 0.533),The Bemegride don,t infect serum ACTH and COR. ④There is notabledifference in GTCS or CP-GTCS and posture seizure groups: ACTH( F=5.989,P= 0.005), COR ( F=4.187, P= 0.022). (2)Compare between groups: There isnotable difference with the serum level of ACTH and COR in case and controlgroup: ACTH( F =7.009, P= 0.011),COR(F=7.477, P= 0.009). 2,The resultsof EEG:(1)EEG changes in the preictal state in sleep epileptic seizures includedthe following: ①Sharp(spike) wave and 3~4Hz sharp (spike)-and-slow-waveand multiple spike-and-slow-wave were detected.②Paroxysmal rhythmic sharp(spike) wave. ③Paroxysmal rhythmic fast wave.④Paroxysmal rhythmic slowwave. ⑤The background activity was unchanged. ⑥The background activitysuddenly changed lower.(2) There isn,t epileptiform discharges in hysteriagroup.Conclusion:1,The level of serum ACTH and COR are close relation with thesleep epileptic seizures . Serum ACTH and COR is lower before and higher inand highest after seizure. 2,The changed level of serum ACTH and COR insleep epileptic seizures are close relation with the focus of infection(especiallyin temporal lobe) and seizure type. 3,The suddenly increased frequency andamplitude and the decreased amplitude of EEG before onset of seizures mightare close relation with the levels of serum ACTH and COR.
