Aim:In order to provide the basis for the formulation of rational use of antibiotic, emphasis the management of antibiotic and promotion of rationally clinical use of antibiotic, the use of antibiotic in the departments of our hospital and its rationality were investigated and analyzed in this paper.Methods:The total income of the medical service, drugs earning and antibiotics use were investigated in 2006. The proportion of drugs accounts for the hospital income and the antibiotics accounts for the drugs were calculated. The first 50 kinds of antibiotics were sorted according to their consumption. The application situation of antibiotics and the injection in the outpatient service were investigated and analyzed based on the spot-checks of the outpatient service prescription in 2006. The application situation of antibiotics in in-patient (including medication variety, medication frequency, administration route and associating situation) and the application situation in surgical operation patient, namely preventive use of the antibiotics in the surgical operation (including medication variety, medication frequency, administration route and associating, surgical department should look up medication opportunity, medication scale, medication time, association situation) were investigated. According to the antibiotics application situation from Jun. 2006 to Jun. 2007, the situation of bacterium picking out and the medicine sensitivity were investigated. The using situation of antibiotics in outpatient service, non-surgery and the surgical operation patient were appraised based on their applications.Result:The income proportion antibiotics account for drugs is 32.06%. The antibiotics is using widely. It refers to 68 kinds of 13 categories in outpatient service, 62 kinds of 16 categories in hospital medical record. The application ratio of antibiotics in outpatient service prescription is 40%. The association rate is 22.53%. In the situation of preventive use in surgery patient, the application ratio is 93.3%, the association rate is 77.1%. In hospitalized non-surgery patient, the ratio is 52.8%, the association rate is 50%. Only 14.7% of the cases take drug sensitivity and etiological examination. The medication plan is incompletely reasonable. The phenomenon of drug abuse exists. In preoperative period, the use of antibiotics is not of specification. From the second half year of 2006 to the first half year of 2007, 3640 samples for examination and test out of all the in-patients have been taken. And 1370 strains have been cultivated in all, including 825 plants of gram-negative bacterium, 156 plants of gram-positive bacterium and 389 plants of epiphyte.There are 6 kinds of gram-negative bacterium and these bacteriums have a high probability of drug fast to manifold antibiotics(except vancomycin and imitenem); There are 11 kinds of gram-positive bacterium been detected, fixedly bacilus have already 24.3% and 29.3% of probability of drug fast to imitenem and meropenem.Conclusion:The sale proportion of antibiotics in our hospital is reasonable on the whole on the high side. There were 3 kinds of antibiotics among the first 10 consumptions in 2006, including Tazidime, injection of cefathiamidine, Cefminox sodium. Cephalosporin is most widely used in our hospital. There is a little illogical case in the application of antibiotics, including the proportion of prescription that cover antibiotics and associating situation of antibiotics. The ratio of applications of antibiotics among the in-patients is on the high side and the check of sensitivity of medication andpathogeny is on the low side. The ratio of applications of the situation of preventive use in surgery patient is a little high. So the unreasonable use of antibiotics still exists in our hospital. The level of reasonable use of antibiotics is expected to be further enhanced in future.
