Purpose We collect clinical data from 388 in - hospital TON patient, to get an epidemiological analysis and evaluate our curative effect with integrated Chinese and Western medicine. Methods We use special table to record patient's data such sex, age, visual acuity and visual field, etc. 388 in-hospital TON patient's case history from 2000 to 2007 in Dongfang Hospital Eye Department are included. All statistical values are dealed with SPSS 14.0.Results Epidemiology All our patients are indirect type of traumatic optic neuropathy, there are 330 males(85.05%)and 58 females(14.95%), 80 patients are bilateral injured, the average age is 26.29 and there are 137 children( below 18) included, Road traffic accidents (61.86%, which contains 39.17% motorcycle accidents), attack(20.62%) and falling injuries (13.92%) were the commonest causes. 221 patients (56.96%)losed consciousness after injury and 69(17.78%)patients with optic nerve canal fracture. For 179 patient who can assure their injury position there are 120 cases(67.04%)with wound in forehead or eyebrow. Only 13 patients(3.35%)start their treatment in our hospital within 7 days of trauma and the average interval time after trauma is 94.12 day. There are 192 eyes with NLP visual acuity before treatment and the average acuity is 2.07(log MAR). Treatment all patients are treated with integrated Chinese and Western medicine with 97 patient accepted therapy of Compound Anisodine Injection, the average time of treatment is 31.49 days. Outcome We set a curative effect standard combined with visual acuity and visual field, and for the totally 468 injured eyes there are 140 eys (29.91%) get a effective score, the average visual acuity after treatment is 2.36(log MAR), the P value of the paired t test is less than 0.001.Conlusion The occurance of TON has a distinct character that male and young person, and workers in some special occupations such as driving and high altitude wok (eg. Architecture worker) have a much higher risk of suffering. We should do something more to avoid the occurring of correlate paroxysmal events and tell people to go to hospital earlier if injured. We also conclude that patients with low acuity before treatment, long interval time after trauma, short time of therapy or Qi-Xue-Liang-Xu TCM type will have a poor outcome.
