| Objective To observe the curative effect of combination of Chinese and Western medicine applied in the treatment of infant hepatitis (bile clot)with yinggan soup and anti-hepatitis-jaundice infection. And explore the mechanism.Methods To choose 48 cases of infants who have received medical treatment in the Chinese Traditional Medicine Department and the digestive system Medicine Department in Wuhan Children Hospital from Mar, 2006 to Oct, 2007, to carry out review statistic analysis and summarization of the documentary. In these cases, the infants have the symptoms of jaundice all over skin, abnormal liver function and expanded liver. divide the 48 infants with hepatitis syndrome into treating group with 28 infants and contrast group with 20 infants randomly. Treat the treating group with yinggan soup and anti-hepatitis-jaundice infection besides the western medicine applied in contrast group ; and treat the contrast group with Western medicine. Measure the main clinic data of the two groups, such as STB, DTB, ALT and the size of liver (below rib/cm), and observe the change of those data.Results1 In the 48 cases of IHS, the sick are all infants whose age are below 12 months. And all the cases are confirmed to be virus infection relevance, among which 19 cases with CMV-IgM positive, the percentage being 41. 3%; 13 cases with CMV-IgG positive, the percentage being 28. 3%; and 12 cases both are positive, the percentage being 26.1%; 8 cases with HSV-IgM positive only, the percentage being 17.4%; 3 cases with rubella virus-IgM positive, the percentage being 6.5%; 2 cases with EBV-IgM positive, the percentage being 4. 3%; 3 cases the cause is not clear, the percentage being 6.5%.2 In these 48 cases, all infants first visited a doctor because of successive yellow skin all over the body or repeated occurrence of it. all infants yellow skin >12mg/L, urine and stool color become deep and deep , stool yellow 6 cases,yellow and white 38 cases,white 2 cases. And the liver function of all infants is affected, and there are jaundice, and abnormal STB, DTB, ALT and the size of liver is below right rib 2.5cm . Other systems of the infants are not infected, such as diseases of the respiratory system, the digestive system and other system,some inborn diseases,diseases of nervous system,effusion of blood and other causes which lead to yellow skin and damaged liver function, such as the bile duct block, GSD.3 The group that receive Chinese traditional and western treatment together, and the group that receive western treatment are similar in the clinic features, seriousness degree, living environment, family economic condition, etc, thus have comparition. after treatment, the data of STB, DTB, ALT of both groups decreased, there is a difference when comparing the two groups P<0. 05-0. 01; and the liver of the infants became smaller, there is a significant difference when comparing the two groups P<0. 05. Curative effect: the recovery ratios of the two groups are 60. 71% and 45. 0% respectively; the general effectiveness ratios of the two groups are 96. 43% and 70. 0% respectively. The general effectiveness ratio of treating group is higher than that of contrast group significantly (P<0. 05).4 In treatment group, in 8 cases with the infants recover, the percentage being 28. 57%, and in 19 cases with the infants get better, the percentage being 67.86%; in 1 case with the infant doesn' t recover, the percentage being 4. 08%; the total effect cases are 27, the percentage being 96.43%. in comparison group, the infants in 3 cases recover, the percentage being 15. 0%; and in 11 cases, the infants get better, the percentage being 55%; in 6 cases, the infants don' t recover, the percentage being 30. 0%; the total effect cases are 14, the percentage being 70. 0%. the effect rate of treatment group noticeably higher than that of comparison group(P<0.05), which indicates that the whole effect of treatment group which receive Chinese traditional and western treatment together is noticeably better than that of comparison group which receive western treatment only. The recovery rate of the two groups is not obviously different (P>0. 05) . Conclusions1 There are multiple causes for IHS, virus infection, bacteria infection, eumycetes,spirochaeta,syphilis infection,heredity diseases and inborn diseases, all might be the cause of this disease. The causes of this disease are really complicated, and the rate of virus infection is the highest. The common causes includeCMV,HBV,rubella virus,EBV,HSVamong which CMV is one of the main cause.2 After remedy, all infants yellow skin <7mg/L, urine and stool color become plain. To improve the symptom and sign of treating group is better than contrast group. The general effectiveness ratio of treating group is higher than that of contrast group significantly. The curative effect of combination of Chinese and Western medicine applied in the treatment of infant hepatitis is remarkable compare with that of contrast group.3 For infants in these 48 cases, they are all newly born babies who are infected by CMV and other virus, which lead to the change of bile, damage and inflation of bile cells. The bile is easily blocked by condense dischange and lead to narrowing of tube and partial blocking of bile In this paper, the practice of combining Chinese traditional and western treatment together to cure HIS infants because of accumulation of gall and partial blocking of bile lead to satisfactory result. It also indicates that the Chinese traditional medicines which benefit liver and bile, could noticeably relieve macro bile infection, maintain the tube through, promote discharging of gall. Carrying out early diagnosis and immediate and effective treatment on infants could prevent it from developing into complete bile block, thus change the prognosis and results of HIS infants.
