Adult Primary Nephrotic Syndrome Glucocorticoid Treatment Before And After Traditional Chinese Medicine Syndrome Evolution Of Law And Integrative Medicine Reducing Toxicity Program

Objective:First,Investigating the evolution law of Traditional Chinese Medicine(TCM) before and after the treatments of primary nephritic syndrome(PNS) with glucocortcosteroid; Second,Evaluating preliminarily the synergism and attenuation of TCM with differentiation step by step on the treatments of PNS with glucocortcosteroid,and preferring treatment of PNS with integrated project of traditional Chinese medicine and western medicine.Methods:The first part,TCM evolution law:formulating unified syndrome observation table, then clinicians recorded the symptom,the objective sign,picture of the tongue and the pulse condition of the patients accurately and truthfully in the clinical observation table according to the experimental request,but type of TCM syndrome was not decided.After the experiments,the person in charge of the experiment determined the type of syndrome according to the unified dialectical standard,and probed into the object law of the evolution of the syndrome of TCM after PNS glucocorticosteroid treatment.The second part,the effects of TCM on the treatments of PNS with glucocortcosteroid:Selecting the primary or recrudescent patients of PNS and divided into a trial group(given TCM and glucocorticosteroids) and a control group(given glucocorticosteroids only) randomly.All the patients underwent a treatment course of 2 months,to compare the difference of clinical remission rate and hormone side effect's incidence rate,evaluating preliminarily the synergism and attenuation of TCM to hormone.Result:First,256 qualified patients were observed,there wre 30 without glucocorticosteroid treatment,50 belong to incipient glucocorticosteroid treatment 1-4 weeks,42 patients with 5-8weeks,83 patients in hormone decrement stage and 51 patients In hormone stage of keeping, analyse the outcome and indicate:TCM evolution law:the type of the syndrome was mostly Qi deficiency and yang deficiency of spleen and kidney before glucocorticosteroid treatment.After the treatment of 1-4 weeks,it was mostly Qi deficiency of spleen and kidney,deficiency of Qi and Yin. After the treatment of 5-8 weeks,it was mostly Yin deficiency of liver and kidney,deficiency of Qi and Yin.In hormone decrement stage,the deficiency of Qi and Yin was mostly pattern.In hormone stage of keeping,it was mostly deficiency of Ying and Yang.As for demonstration,the accompanying symptoms were mostly retention of water and turbid damp before using the hormone.After the treatment of 1-4 weeks,the accompanying symptoms were mostly retention of water,turbid damp and blood stasis.Miscellaneous accompanying symptoms were very conspicuous after the treatment of 5-8 weeks.In hormone decrement stage,it was mostly turbid damp,blood stasis,humid heat and pathogenic factor of wind.In hormone stage of keeping,blood stasis was mostly pattern.During the whole hormone treatment,turbid damp,humid heat and blood stasis were very obviously.Second,the effects of TCM on the treatments of PNS with glucocortcosteroid:there are 91 qualified patients,the trial group have 45 patients and the control has 46 patients among the total,the result indicate,the improvements of clinical symptoms and laboratory indexes of the trial group were obviously greater than those of the control group.For the side effects of glucocorticosteroids,the respective incidence rate of acne,insomnia,menoxenia and hypertrichiasis was predominantly lower in the trial group.There was no significant difference in the incidence rate of extremity cramps and Cushing's syndrome.Conclusion:First,TCM evolution law:when the patient of PNS was treated with hormone, dynamic change with type of traditional Chinese medicine could be observed.As for deficiency pattern,the changed law was the Yang deficiency of spleen and kidney or the Qi deficiency of spleen and kidneyâ†'the Qi deficiency of spleen and kidney or the deficiency of Qi and Yinâ†'the Yin deficiency of liver and kidney or the deficiency of Qi and Yinâ†'the deficiency of Qi and Yinâ†'the deficiency of Yin and Yang.As for demonstration,the changed law was water wet and turbid dampâ†'water wet,turbid damp and blood stasisâ†'blood stasis,humid heat,convulsion, turbid damp and water wetâ†'turbid damp,humid heat and blood stasisâ†'the stasis of blood.During the whole hormone treatment,turbid damp,humid heat and blood stasis were very obviously. Especially,blood stasis was observed through course of disease.Second,the effects of TCM on the treatments of PNS with glucocortcosteroid:The project of traditional Chinese medicine and western medicine with differentiation step by step on the treatments can raise hormone's effectiveand mitigate it's side effect.
