Cor Pulmonale Acute Exacerbation Of Literature Research And Western Combined With A Summary Of The Treatment Program On The Clinical Efficacy Of Pulmonary Heart Disease

Objective:To evaluate the effectiveness of comprehensive treatment of Traditional Chinese Medicine combining with Western Medicine for SPHD during acute period through radom,comparative experiment research,based on earlier stage work.And this research attempts to formulate one comprehensive treatment of Traditional Chinese Medicine with Western Medicine for SPHD during acute period, which can not only enhance clinical curative effect,but also fit for Chinese conditions.Methods:The patients suffering from SPHD during acute period and differentiation for phlegm remaining in lung and obstruction of lung qi are randomely divided into treatment group(receiving comprehensive treatment of Traditional Chinese Medicine with Western Medicine) and control group(receiving conventional treatment of Western Medicine).The treatment group receives Traditional Chinese Medicine adding to conventional treatment of Western Medicine.The basic syndrome is obstruction of phlegm stagnating in lung,so the treatment principle is dispersing lung-qi and relieving asthma,resolving phlegm to stop coughing.The formulas are San ao decoction,Kuo lou xie bai ban xia decoction and Jie geng decoction.If the patient has deficiency of lung and Spleen,we add Xiang sha liu jun zi decoction to strengthen spleen and tonify lung.If the patient has edema due to Yang Deficiency,we add up Wu-Ling decoction to warm Yang and induce diuresis.The control group receives conventional treatment of Western Medicine with placebo of Chinese Medicine.The conventional treatment of Western Medicine includes controllable oxygen inhalation, anctive anti-infection,keeping respiratory tract smooth,improving ventilation, rectifying lack of oxygen and carbon dioxide stagnation,electrolyte and acid-alkaline-balance disturbances,reducing high pulmonary arterial pressure, rectifying congestive heart failure,nutritional support,preventing and treating complications.Both of the groups are treated for 14 days.The effect observed indexes are as follows:The general curative effect,integral of main and secondary symptoms of TCM,integral of western medical signs,white cell total and neutrophil classification,arterial blood gas analysis,evaluating of personal living abilities,etc.Result:The treatment group has quicker effect on reducing moist rale and resisting infection,comparing with the control group,but they have the same effects at terminal point.From the middle point to terminal point,the treatment group has better effect than the control group on improving symptoms,signs and laboratory examination,such as coughing,expectoration,cyanosis,respiration rate,edema,PO2, etc.The treatment group has better effect on improving PCO2 at terminal point.The ratios of clinical control,significant effect and the general curative effect of treatment group are higher than those of control group,but without statistical significance,it maybe due to small number of statistical samples.The personal living abilities of treatment group are better than those of control group one month after the terminal point,it has statistical significance.Conclusion:The comprehensive treatment of Traditional Chinese Medicine combing with Western Medicine has quicker effect on resisting infection,better effect on improving main symptoms,signs when SPDH is in acute period.It also has better effect on personal living abilities one month afrer the terminal point,comparing with comprehensive treatment of Western Medicine.There is no statistical significance on ratios of significant effective and the whole curative effect between the two comprehensive treatment groups.It is need to increase sample quality for future study.
