| Objective: In the clinical practice for years, we raise that the main pathological characteristics of CS is asthenia in origin and sthenia in superficiality, that is to say, insufficient liver- and kidney-yin and deficient blood and qi as the root, the obstruction of blood vessels by pathogenic factors such as cold, wind, and dampness as the superficiality according to the pathogenic features and clinical manifestations of CS. Based on these theories, Guben Shujin Decoction, a self-formulated decoction in treatment of cold dampness type CS has been clinically used for years, with better therapeutic effects obtained. Now it has become a common recipe in our department in treatment of CS. By retrieving literatures, the recognition and treatment of CSR with TCM and modern medicine were systematically reviewed in this paper. We suggest using Guben Shujin Decoction in treatment of CSR. And the clinical value of this method in treatment of CSR was explored.Methods: Observation of the therapeutic effect of Guben Shujin Decoction was conducted in this study using a randomized and controlled clinical method. The clinical efficacy of Guben Shujin Decoction in treatment of CSR was observed. Each subject was strictly screened referring to a uniform diagnostic standard, exclusion standard, and inclusion standard. Gentongping Granules, a well-recognized control medicine with reliable clinical efficacy was used as the control. Sixty patients with CSR orally took self-formulated Guben Shujin Decoction and Gentongping Granules from July 2007 to December 2008. The observatory tables were scaled according to patients' main symptoms and signs. The pre- and post-treatment integrals were filled according to the observatory items. The observatory contents cover the improvement of pain, the improvement of the radiating pain to the upper limbs, the improvement of the tenderness of the cervical vertebrae, the improvement of the neck motion range, and the improvement of numbness, and so on. Each patient was scored before treatment and 2 weeks after each treatment.Results: The good clinical rate and the total effective in the treatment group were superior to those in the control group. The statistical analysis showed according to the scoring methods for symptoms and signs, 10 cases (accounting for 33.3%) had the improvement rate more than 80%; 14 cases (accounting for 30%) with the improvement rate ranging 65%-80%; 4 cases (accounting for 13%) ranging 50%-65%; 2 cases (accounting for 6%) less than 50%. The good clinical rate was 80% and the total effective rate was 93. 3%. The scores of symptoms and signs in the 30 patients treated with Guben Shujin Decoction were significantly reduced. showing significant difference after paired t-test (P<0. 05). The statistical analysis showed that the treatment group was superior to the control group in the improvement of symptoms and signs. VAS was used by patients to conduct self scoring on the pain degree. The pre- and post-treatment results showed significant statistical difference by paired t-test (P<0.05), but with no statistical significance when comparison of intra-group difference by paired t-test (P<0.05), indicating obvious pain relief effects in the two group. Guben Shujin Decoction showed better effect in improving symptoms and signs than Gentongping Granules.Conclusion: Guben Shujin Decoction, a self-formulated recipe in treatment of CSR referring to the treatment principle of disease identification and syndrome differentiation. On the basis of fully recognition of the pathogenesis of CS, it targets the pathogenesis and shows better therapeutic efficacy of cold dampness CS. It may effectively relieve pain and improve accompanied symptoms. It is safe and effective in clinical use. But its pharmacological mechanism of action awaits further studies. |
