Objective With the change of life style and habit of the diet,the number of the patients who have various risk factors increases obviously.This group have more danger to coronary heart disease.How can we cull the patients who gather high risk factors in this throng as early as possible and give an enhance to intervene are the demands of the prevention of cardiovascular disease.Studies have found that adipocytes produce and secrate a variety of proteins which influence the structure of organize and function conceptualized as adipocytokines.Adiponectin is a recently discovered adipocytokine which is expressed in white adipose tissue.Recent studies show that Adiponectin might play important roles in the occurrence and development of coronary heart disease.The objective of this study is to explore the relationship between serum adiponectin and the degree of coronary artery stenosis in middle-aged male patients and the predictive values of adiponectin for coronary artery disease.Methods The serum adiponectin levels were measured by ELISA in 86 middle-aged male patients with coronary heart disease(coronary stenosis more than50%),32 middle-aged male patients with coronary atherosclerosis(coronary stenosis less than 50%),and 30 control subjects with normal coronary artery.Meanwhile BMI,TG,TC,FPG and FINS were measured and HOMA-IR index were calculated.The degree of artery stenosis was confirmed by coronary angiographies and assessed quantitatively with Gensini score.Result The serum adiponectin concentration in middle-aged male patients with coronary atherosclerosis(ASO)and coronary heart disease(CHD) was signifieantly lower than that of control group respectively(P<0.01);Serum concentration of adiponectin in CHD patient s was significantly lower t han that in ASO(P<0.05);Serum concentration of adiponectin in ACS patient s was significantly lower than that in SAP(P<0.01).The serum adiponectin concentration in Middle-aged Male Patients with triple,double and single vessel disease was signifieantly lower than that of control group respectively(P<0.01).Adiponectin of triple vessel disease was signifieantly lower than that of double and single vessel disease respectively(P<0.05).As the Gensini score increased,the adiponectin levels decreased(P<0.05).Serum adiponectin levels correlated negatively with BMI,LDL-C(r=-0.291,-0.221,P<0.05) and HORA—IR index(r=-0.476,P<0.01).Multiple linear regression showed that HORA—IR index was independently correlated with the concentration of adiponectin level in the middle-aged male patients with CHD(r~2=0.46,P<0.01)。Conclusion The serum adiponectin levels were related closelv to the degree of coronary artery stenosis in the middle-aged male patients with coronary atherosclerosis and CHD.It reflects the stability of coronary artery stenosis which may predicts the occurrence and development of coronary artery disease.The finding suggests that adiponectin might be involved in the pathophysiologic process of coronary atherosclerosis and be used to estimate the degree of coronary artery stenosis.
