Hepatic myelopathy is a rare neurological complication of chronic liver disease which the pathophysiological mechanism in our patient has not been established.It mainly caused by myeleterosis after portosystemic shunt operation and portosystemic collateral circulation.The pathology was demyelinating on pyramidal tracts of spinal cord.The clinical manifestation is that two legs spasm paraplegia step by step.There are three hypothesis contain the theory of chronic intoxication,the theory of malnutrition,the theory of immunologic factors.To Chinese medicine,there are not correspondence name. From the symptom,it owns to "Flaccidity Zheng","Meteorism","Jaundice", "Stroke".The method of treatment which my mentors used should focus on the principal cause of a disease and the theory of syndrome differentiation and treatment.He thought the main location of disease was in liver,spleen and kidney.It was caused by the deficiency of kidney and liver and the circulation of blood blocked.It still can aggravate to liver wind stirring,hemopyretic bleeding and bleeding due to Qi deficiency.So treat this disease my mentors used two methods about invigoration liver and kidney and live a blood net all long.Accordance with the theory of liver and kidney homology,cure stagnant blood must cure Qi,cure paralysis to only take Yangming,the resemblance theory of chinese herbal drugs and so on to treat hepatic myelopathy.Then evidence of different symptom to add or less Chinese drugs to cure disease.At the same time,combine a modern medical result in hepatic myelopathy research, consociation with medicine,in the adoption the doctor combine of treatment method,to improve the clinical effect.
