Objective The point mutation of BRAF and the iodine nutrition may impact the occurrence and development of thyroid carcinoma. To investigate the trend of thyroid papillary carcinoma(PTC),follicular carcinoma(FTC), medullary carcinoma(MTC) and undifferentiated carcinoma(ATC) during 11 years and the frequency of BRAF mutation in PTC both in the cities of Qingdao and Linyi, and make the comparative analysis between them, in order to discuss the relationship between the mutation of BRAF and iodine nutrition and thyroid.Methods (1) The samples were accessed from Qingdao and Linyi from 1998 to 2008. According to international tumor classification standards, the retrospective method was used to analyze the average diagnosed age, sex ratio and the ratio of annual thyroid cancer in total. (2) The BRAF T1799A mutation was detected by polymerase chain reaction (PCR) and direct sequencing method in 220PTCs(110 cases of Qingdao and 110 cases of Linyi) that had been fixed in formalin and embedded in paraffin. The statistical analysis of the relationship between the mutation and the clinical pathology was carried out by SPSS 13.0.Results (1) The total of thyroid cancers in 11 years in Qingdao were 734, of which PTC accounted for 85.56%, FTCaccounted for 8.86%, MTC accounted for 4.36%, ATC accounted for 1.22%. The ratio of male to female was 1:1.25-3.94. The average diagnosed age of PTC was the lowest(45.39 years), and the ATC was the highest(61.56 years). While the total of thyroid cancers in Linyi were 421, of which PTC accounted for 82.90%, FTC accounted for 9.74%, MTC accounted for 4.75%, ATC accounted for 2.61%. The ratio of male to female was 1:2.33-7.20. The average diagnosed age of PTC was the lowest(42.09 years), and the ATC was the highest(67.91 years). Both in Qingdao and Linyi, the ratio trend in PTC was continuously growing year by year. The data of FTC showed uncontinuously increasing trend more or less. While the increasing trend was not obvious in MTC and ATC. (2) The percentage of BRAF T1799A mutation in PTC was 41.82% in Qingdao and 30.91% in Linyi(P>0.05). The clinical stages (Ⅲ/Ⅳperiod)of PTC in Qingdao in the BRAF (+) was 47.83%, BRAF (-) was 20.31%; that was 41.18% and 22.37% in Linyi. Both of them have Significant differences(P<0.05). There was no significant correlation between BRAF mutation and age, gender, lymph node metastasis and with lymphocytic thyroiditis(P>0.05). Conclusion During the 11 years, the ratio trend in PTC both in Qingdao and Linyi was continuously growing year by year, while that was not obvious in FTC,MTC and ATC. The percentage of BRAF T1799A mutation in PTC between Qingdao and Linyi has no sense in statistics. There was significant correlation between BRAF mutation and PTNM. The relationship between BRAF mutation and age, gender,lymph node metastasis and prognosis remains to be further studied.
