Objective:To observe the short-term effect of arterial stiffness in hypertensive patients treated with amlodipine, bisoprolol, perindopril or telmisartan.Methods:80 cases of essential hypertensive patients were collected from the out-patient clinic of the Third Xiangya Hospital of Central South University. Randomly divided them into four groups:amlodipine group(A group, n=20):5mg amldipine besylate tablets; bisoprolol group(B group, n=20):5mg bisoprolol fumarate tablets; perindopril group(C group, n=20):4mg Perindopril capsules; telmisartan group (D group, n=20) 40mg Telmisartan tablets; Meanwhile, Selected 20 cases of people with normal blood pressure as a control group (E group, n=20):Placebo capsules. Drugs were given with patients at eight AM in all groups via oral administration. Heart rate, blood pressure, height, weight, blood urea nitrogen, creatinine, uric acid, pulse wave velocity (baPWV), and ankle-brachial index (ABI) were measured, Venous blood specimens were collected before taking medicine. The same indexes were observed again at three hours later after taking medicine. Using Radioimmunoassay to detect plasma endothelin concentrations before and after treatment, simultaneously, plasma nitric oxide concentration was analyzed by colorimetric.Results:1. Before treatment, all indexes between each group except control group showed no statistically significant differences (P> 0.05).2. Changes of blood pressure and Arterial stiffness in each group:In treatment groups, systolic blood pressure, diastolic blood pressure and pulse pressure differential values were significantly declined (p< 0.05); The declined level of systolic blood pressure of A group was more lower than D group (P< 0.05), however, the declined level of systolic blood pressure between other treatment groups showed no statistically significant differences (P> 0.05); After intervention, analyzed all indexes between each treatment group and the control group indicated statistically significant differences (p< 0.05).After Short-term Antihypertensive therapy a significantly declined in the baPWV mean differences of each group was observed(p< 0.05), the rates of baPWV declined in each group was:241.5 cm/s (A group), 159.5 cm/s(B group),157.5 cm/s (C group) and 85 cm/s (D group); and baPWV declined level of A group was significantly greater than D group (P< 0.05), however, baPWV declined level between other treatment groups showed no statistically significant differences(P> 0.05); Compared with control group, the baPWV and the change value in each treatment group were statistically significant differences after treatment (P<0.05).3. Changes of endothelin and nitric oxide in each group:After treatment, it was observed that endothelin were significantly declined in each treatment group(P< 0.05),except D group(P=0.244); After the treatment group intervention nitric oxide mean difference value is increased (p< 0.05), groups was statistically significant (p> 0.05). Compared with control group endothelin, nitric oxide index change both not significant (P> 0.05).4. After Short-term antihypertensive treatment,△SBP,△DBP,△PP was positively correlated with△baPWV (r= 0.523,0.283,0.391; p= 0.000,0.004,0.000);and endothelin (r=0.274,0.262; P=0.006,0.008),while negatively correlated with△NO (r=-0.239, P= 0.003; r=-0.320, P= 0.000).Conclusion:1. Benzene sulfonic acid amlodipine, bisoprolol, perindopril,, telmisartan short-term intervention in lower blood pressure, also can improve the artery baPWV, among them with amlodipine group is more obvious.2. Different hypotensive short-term intervention shows can improve endothelial function.
